The efficacy and QOL in a cancer pain patient who converted to transdermal fentanyl from low-dose morphine (n=5) or high-dose morphine (n=5) were retrospectively compared. Cancer pain control was evaluated by visual analogue scales (VAS) score. Patients switched from low-dose morphine demonstrated significantly better scales. The number of morphine rescues was also significantly less in the low-dose conversion group. In conclusion, conversion from low-dose morphine is useful for cancer pain control from the aspect of efficacy and QOL.

Corticosteroid signaling plays a critical role in modulating the neural systems underlying reward and addiction, but the specific contributions of glucocorticoid receptors (GRs) and mineralocorticoid receptors (MRs) in the medial prefrontal cortex (mPFC) to opioid reward and dopaminergic plasticity remain unclear. Here, we investigated the effects of intra-mPFC injection of corticosteroid receptor ligand (corticosterone; CORT), glucocorticoid receptor antagonist (RU38486; RU), and mineralocorticoid receptor antagonist (spironolactone; SP) on morphine-induced conditioned place preference (CPP) and dopamine transporter (DAT) expression in the mPFC. While low-dose cannabinoid 2 (CB2) receptor agonists attenuate morphine tolerance in cancer pain models, chemokine ligand 12 (CXCL12)/chemokine receptor 4 (CXCR4) expression induces morphine tolerance.
