Changes of the lid fissure after surgery on horizontal extraocular muscles.

Published: December 2005

Purpose: To evaluate the effect of isolated medial and lateral rectus muscle surgery on eye lid position.

Design: Prospective, interventional case series.

Methods: We recruited 36 patients with strabismus with surgery performed on one horizontal extraocular muscle only. The width of the lid fissure and the position of the upper and lower lids in relation to the pupil center were measured before and 3 months after surgery. Regression analysis was used to evaluate the surgical effect.

Results: Recessions induced a widening of and resections a narrowing of the lid fissure, mainly because of changes of the lower lid position. The effect was statistically significant (change in lid fissure width, = 0.13 x surgical dose in millimeters; r(2) = .35; P = .0001).

Conclusion: Patients should be informed about changes in lid fissure width after operations on horizontal extraocular muscles. If adequately integrated into the surgical plan, this side effect may alleviate pre-existing interocular differences in lid configuration.

Purpose: To describe a surgical method for correcting lower lid entropion, lateral canthal entropion, and macroblepharon.

Methods: Lid margins were incised at a 45° angle, and lateral lid margins and a rhomboid shaped piece of skin were resected based on the degree of macroblepharon. Subcutaneous tissue was sutured with absorbable sutures and anchored to the orbital ligament with a non-absorbable suture to maintain lateral canthal position.

Purpose: Euryblepharon is a rare congenital eyelid malformation characterized by symmetrical horizontal enlargement of the palpebral fissure. The eyelid is shortened vertically compared with the horizontal dimension. The lateral canthus is most commonly affected.

This report details the case of a middle-aged man with chronic unilateral total limbal stem cell deficiency with severe symblepharon in his left eye with upper lid mechanical ptosis, restricted ocular movements and visual acuity limited to counting fingers at 2 feet. 9 months prior to his first visit, he sustained an accidental unilateral alkali injury to the left eye. He underwent autologous simple limbal epithelial transplantation combined with conjunctival autograft in the left eye.

November 2024

Department of Ophthalmology, Layton Rahmatullah Benevolent Trust (LRBT) Karachi-Pakistan.

Background: Reconstruction of the lids is a significant challenge for ocular plastic surgeons as it requires the greatest functional and cosmetic outcomes. This study aimed to share the experience of Tenzel rotational flap in upper and lower lid reconstruction of anterior lamella at a tertiary eye care hospital of Karachi, Pakistan.

Methods: 10 patients with upper and lower eyelid defects over a period of 6 months were a part of this prospective interventional clinical study.

Article Synopsis
  • Enophthalmia, characterized by abnormally sunken eyeballs and associated eyelid issues, poses both functional and aesthetic challenges for patients, leading to a demand for improved surgical techniques to rejuvenate the periorbital area.
  • The study introduces a modified transconjunctival lower blepharoplasty (TCLB) that incorporates additional de-flaming and decompression maneuvers, resulting in enhanced eye-opening capabilities and a reduction of visible symptoms in both upper and lower eyelids.
  • After analyzing the results from 40 patients, statistical data indicated significant improvement in palpebral fissure height (PFH), confirming that the modified TCLB led to more effective surgical outcomes compared to traditional methods.
