Aim: We evaluated the impact of impermeable bed covers on asthma in asthmatics with clinically relevant house dust mite (HDM) sensitization.

Methods: The study included 32 HDM-sensitized asthmatics in whom HDM allergy was considered as a significant factor in their asthma. They were randomized into either an intervention group whose bedding was encased with impermeable covers, or a control group who received cotton covers. Before and 3 and 6 months after encasement, dust samples were collected from the bedding and assayed for Der p 1. Clinical outcomes included quality of life, lung function, bronchial reactivity to methacholine, symptoms, medications and peak flow rates.

Results: Baseline Der p 1 levels in both the active and the placebo groups were comparable and high (19.2 vs 18.9 microg/g of dust). There was a significant reduction in Der p 1 levels in the active group after 6 months, but not in the placebo group (7.3 vs 21.9 microg/g of dust). Quality of life improved significantly in both the intervention and control groups, but there was no significant difference in the improvements between the groups. There was no significant change in lung function, symptoms, and requirements for medications.

Conclusions: Encasement of bedding significantly reduced the Der p 1 levels. However, this was not sufficient to produce worthwhile clinical improvement in those in whom dust mite avoidance might well have been recommended as part of their clinical management.

This article provides a comprehensive overview of the current treatment options for patients with metastatic castration-resistant prostate cancer (mCRPC) following the failure of first-line therapy. Although significant progress has been made in the primary treatment of hormone-sensitive prostate cancer, the management of mCRPC remains a clinical challenge. The article outlines the diagnostic criteria for mCRPC, which can be confirmed through biochemical progression and imaging techniques.

Temperate streams are subsidized by inputs of leaf litter peaking in fall. Yet, stream communities decompose dead leaves and integrate their energy into the aquatic food web throughout the whole year. Most studies investigating stream decomposition largely overlook long-term trajectories, which must be understood for an appropriate temporal upscaling of ecosystem processes.

Science is crucial for evidence-based decision-making. Public trust in scientists can help decision makers act on the basis of the best available evidence, especially during crises. However, in recent years the epistemic authority of science has been challenged, causing concerns about low public trust in scientists.

Background: Due to age-related changes the lung function decreases. At the same time there is an increase in pulmonary diseases that lead to restrictions in mobility and autonomy.

Research Question: What are the underlying changes in lung ageing? To what extent do they affect lung function and are there factors that can be influenced?

Method: Literature search.

[Perioperative management of obese patients undergoing elective hip and knee arthroplasty].

Klinik für Orthopädie, Allgemeines Krankenhaus Linz, Linz, Österreich.

Introduction: Obesity is a worldwide growing health crisis, affecting younger and younger age groups and posing new challenges for arthroplasty. Due to increased complication rates in this population, many authors are of the opinion that in patients with a body mass index (BMI) of over 40, the problems outweigh the benefits of surgery and that surgery should be postponed until significant weight reduction has been achieved.

Materials And Methods: A systematic review of electronic databases (PubMed, Embase, Web of Science and the Cochrane Library) was performed.
