[Is intrinsic or non-atopic asthma a particular disease?].

Rev Med Suisse

Service de pneumologie, HUG, 1211 Genève 14.

Published: November 2005

Intrinsic or non-atopic asthma is defined by the absence of positive skin test for specific aero-allergens, together with the absence of specific serum IgE. In the adult population, about one third of asthmatic patients are non-atopic. Non-atopic asthma begins generally later in life and is often more severe than atopic asthma. It is more often associated with chronic sinusitis and nasal polyps, a fact that is often underestimated. At the cellular and molecular level, non-atopic asthma and atopic asthma a very similar, and the reason for their clinical difference is not really understood.

Article Synopsis
  • The study investigates the fractional exhaled nitric oxide (FeNO) levels in a healthy population to better understand normal ranges and their influencing factors.
  • FeNO levels were measured in 2,251 participants aged 6-82 years, revealing median values that increase with age and vary based on gender, height, and eosinophil counts.
  • The findings establish reference ranges for FeNO that can aid in clinical asthma diagnosis and management.

Background: Asthma in the elderly is usually considered homogeneous and non-atopic.

Objective: To compare clinical, functional and immunological features between elderly asthmatics with long-standing asthma (LSA) and those with late-onset asthma (LOA).

Methods: Eighty-two asthmatics older than 64 were included into LSA (asthma onset before age 40;  = 46) and LOA (asthma onset from 40 years of age on;  = 36) groups.

Background: Periostin and human chitinase-3-like protein 1 (YKL-40) have been suggested to be involved in the development of airway fibrosis and remodeling. This study aimed to investigate the relationship between serum periostin levels and airway hyperresponsiveness (AHR) and between serum YKL-40 levels and AHR in children with asthma, comparing periostin as a marker for Th2 inflammation and atopy with YKL-40.

Methods: The study involved children aged 6-15 years, comprising 75 with asthma and 29 healthy controls.

Article Synopsis
  • Primary atopic disorders (PAD) are rare genetic conditions caused by specific gene variants that affect skin and immune function, making diagnosis challenging among common allergic disease cases.
  • Identifying PAD requires recognizing clinical red flags like family history and unusual infections, as conventional lab tests are inadequate for definitive diagnosis.
  • Whole-genome sequencing (WGS) enhances diagnostic efficiency and accuracy, but requires careful interpretation and collaboration among specialists to effectively manage PAD cases.

Article Synopsis
  • Allergens, like dust or pet dander, can make people allergic, and in Korea, about 45% of people are sensitized to them.
  • The study tested over 2,300 people to see how allergic reactions relate to age and found different patterns for young people and older adults.
  • People who are sensitized to allergens have higher chances of getting allergic conditions like skin rashes and sinus issues.
