Background: Pediculolysis is bone hypertrophy and pseudoarthrosis caused by pedicle fracture and has often been combined with contralateral spondylolysis in previous reports. Multilevel pediculolysis with spondylolysis is extremely rare, and we report a case who underwent surgery. Cases of multisegment pediculolysis were reviewed to inspire the diagnosis and treatment of similar pathological phenomena.

Although neurovascular structures, including the superior gluteal artery, sciatic nerve, obturator nerve, internal iliac vein and artery, and lumbosacral plexus, are at risk when S2 alar-iliac (S2AI) screws are used, no cases of nerve injuries have been reported. An 84-year-old man was referred to our institute with persistent left sciatica for seven months after undergoing salvage surgery using S2AI screws for deep surgical site infection from a previous posterior interbody fusion surgery at L5-S1. Based on the radiographic and diagnostic selective nerve root block findings, a diagnosis of left L5 radiculopathy was suspected due to the left S2AI screw being caudally misplaced and severely protruding into the pelvic cavity.

Unlabelled: Peripheral malignant nerve sheath tumours (MPNST) are rare and aggressive soft tissue sarcomas often associated with neurofibromatosis type I (NF-1). We describe a case of 26-year-old female with NF-1, initially misdiagnosed as sciatica, who was later found to have a malignant spindle cell peripheral nerve sheath tumour of the right sciatic nerve. Magnetic resonance imaging (MRI) proved to be a valuable tool for diagnosis.

Side effects are usually mild, and the class as a whole is generally well-tolerated by patients. However, one rare and more serious side effect is statin-induced autoimmune necrotizing myopathy.
