Objectives: To point out the efficacy of supra trigonal cystectomy with orthotopic substitution iliocystoplasty in the treatment of advanced interstitial cystitis (IC).
Methods: We study the results obtained in four women suffering IC nonrespondent to conservative treatment who underwent the procedure. All patients comply with classic diagnostic criteria of IC, presenting long-lasting symptoms, between 4 and 8 years (mean 5.6), and voiding frequency of near one-hour day and night.
Results: Mean postoperative follow-up was 32 months (18 to 56); post operative evaluation included clinical evaluation, ultrasound, urodynamic studies, and radiological tests. Suprapubic pain disappeared in all cases, as well as pre-op lower urinary tract symptoms, with good control of urinary frequency day and night being evident in the immediate postoperative period. All patients referred high satisfaction with the outcome.
Conclusions: When conservative treatment fails, supratrigonal cystectomy with orthotopic neobladder substitution is a valid therapeutic option in IC patients who comply with classic diagnostic criteria.

Introduction And Importance: Orthotopic neobladder reconstruction may be associated with complications such as neobladder-vaginal fistula (NVF) and uretero-ileal anastomotic stricture which are often managed using stents, though these can be prone to migration.
Case Presentation: We present a 69-year-old lady with a surgical history of radical cystectomy and ileal neobladder that was complicated by a left ureteric stricture and poucho-vaginal fistula for which she underwent a left ureteric stent and bilateral percutaneous nephrostomy (PCN) insertions after which she lost to follow-up. The patient later presented to our center with a right subcostal anterior abdominal wall desmoid tumor for which she underwent wide local excision.

The article presents a clinical observation demonstrating the functional results of orthotopic bladder plastic surgery according to Studer after radical cystectomy, which demonstrates the progression of chronic kidney disease (CKD) 2.5 years after surgery. According to the literature, patients who survived the early postoperative period with muscle-invasive bladder cancer and no lymph node involvement have a high probability of relapse-free and cancer-specific life expectancy of more than 15 years, but the probability of developing severe renal failure reaches 20%, which reduces the quality and duration of life of such patients.

The patient presented 4 months after laparoscopic radical cystectomy with intracorporeal orthotopic ileal neobladder, with fever and left lower back pain.Laboratory analysis showed leucocytosis and elevated C reactive protein and creatine levels.
