Article Synopsis
  • Angiodysplasia (AD) is a vascular condition in the gastrointestinal tract that often causes lower GI bleeding, especially in the elderly, and can be challenging to detect during endoscopy.
  • An 82-year-old woman with recurrent melena and anemia underwent various evaluations including contrast-enhanced CT scans, leading to a diagnosis of colonic AD after surgery.
  • The case emphasizes that contrast-enhanced CT is a safer and more convenient diagnostic tool for identifying AD-related bleeding in frail, elderly patients compared to traditional methods.

Background: Gastrointestinal (GI) bleeding is common among hemodialysis (HD) patients. Endoscopic examination of the upper and lower GI tract often fails to identify hemorrhagic lesions in anemic HD patients. The study aims to evaluate lesions of the small bowel mucosa in HD vs non-HD patients with suspected small-bowel bleeding (SSBB) using capsule endoscopy (CE) after negative upper and lower GI endoscopies.

Article Synopsis
  • Heyde syndrome consists of anemia, aortic valve stenosis, and acquired von Willebrand syndrome, yet it remains under-recognized in clinical settings.
  • A 70-year-old woman with severe iron-deficiency anemia and aortic stenosis was diagnosed with Heyde syndrome after extensive testing that ruled out other bleeding sources.
  • Aortic valve replacement surgery improved her anemia and symptoms, highlighting the need for awareness of Heyde syndrome in cases of unexplained anemia, particularly when associated with aortic stenosis.

Heyde's syndrome (HS) represents an association between aortic stenosis and intestinal angiodysplasias, and it has been demonstrated that acquired von Willebrand disease plays a pivotal role in the pathophysiology of this syndrome. In patients with HS, von Willebrand factor deficiency represents an additional risk factor, further contributing to the risk of bleeding and anemia. We present the case of an 86-year-old patient diagnosed with HS and von Willebrand deficiency in 2018.

Capsule endoscopy for small bowel bleed: Current update.

Indian J Gastroenterol

October 2024

Institute of Gastrosciences and Liver Transplantation, Apollo Multispeciality Hospitals, Kolkata, 700 054, India.

Small intestine, hitherto an obscure area for endoscopists before 2000, is now easily evaluated non-invasively using capsule endoscopy and invasively by device-assisted enteroscopies. Major advances in understanding the causes and management of small bowel diseases have been in obscure gastrointestinal (GI) bleed, currently re-named as small bowel bleed, after the discovery of capsule endoscopy. The current article is a narrative review of the technology of capsule endoscopy, its advantages and limitations, future perspective and Indian studies on its utility in patients with small bowel bleed.
