Background: In the surgical approaches to the posterior fossa, the accurate location of the transverse-sigmoid sinus (TS-SS) complex is of great importance. The asterion is a referral landmark to the transverse sinus location.

Method: Twenty-five skulls of adult cadavers were studied. We seek for the relationships of the asterion with: TS location, mastoid emissary vein, suprametal crest and inion.

Results: The asterion was found in 49 cases. In the great majority of cases (87.8%) the asterion was over the TS (72.2% over the sinus proper, 27.8% over the TS-SS transition). The mastoid emissary vein was present in 46 cases, and in 36% we found two veins.

Discussion And Conclusions: The burr hole for posterolateral approaches to the posterior fossa must be located below and behind the asterion.

Patients with pectus excavatum and scoliosis can present a unique clinical challenge to operative correction. In patients with severe deformities, vascular structures in between the spine and sternum are at risk of compression, leading to hemodynamic collapse during correction of a spine deformity in the setting of unrepaired pectus excavatum. Careful consideration and multidisciplinary coordination should be used to determine the optimal timing, sequence, and operative approach in repair of the anterior and posterior deformities.

The subsuperior segment (S∗), as an atypical pulmonary segment, is located between the superior and posterior basal segments. Many thoracic surgeons have no experience with anatomical segmental resection of S∗. We present a case of the posterior approach of left S∗ segmentectomy.

Biventricular Dysfunction Due to Chronic Mitral Valve Regurgitation Caused by Aberrant Mitral Arcade.

Ann Thorac Surg Short Rep

December 2024

Division of Cardiac Surgery, Department of Surgery, Thomas Jefferson University, Philadelphia, Pennsylvania.

A 53-year-old male individual with chronic severe mitral regurgitation presented with biventricular dysfunction, pulmonary hypertension, and atrial fibrillation. Echocardiography demonstrated a posterior leaflet prolapse with malcoaptation. Mitral valve repair and Maze procedure were performed, revealing absent chordae and direct connection from the anterolateral papillary muscle to the posterior leaflet, consistent with partial mitral arcade.

Submitral aneurysm is a challenging and uncommon cardiac disease that is uniquely related to the posterior mitral valve leaflet. Awareness and prompt identification are vital because of the strong predilection for sudden fatal complications. Techniques and timing of surgical procedures are not standardized, especially in incidentally detected cases.
