Objective: To describe a rare clinical presentation of necrosis of a parathyroid adenoma, manifesting as fever of unknown origin.

Methods: We present a case report, including detailed clinical and laboratory data, of a patient with previously undiagnosed primary hyperparathyroidism in whom necrosis of a parathyroid adenoma developed during a psychiatric admission for depression.

Results: A 40-year-old woman was admitted with severe depression. During her hospitalization, she was found to have osteitis fibrosa cystica, pathologic fractures of her left femur, and primary hyperparathyroidism. Shortly after her admission, spontaneous necrosis of her parathyroid adenoma developed, in association with high-grade fever, acute hypercalcemic crisis, neck pain, hypophonia, and shortness of breath. Findings on extensive evaluation of the fever remained unremarkable, with persistent high-grade temperatures that resolved postoperatively.

Conclusion: Spontaneous necrosis of a parathyroid adenoma is a rare but well-documented complication of primary hyperparathyroidism. Fever, a previously undocumented associated finding, might be a part of the clinical syndrome of this complication.

Uric Acid Correlates with Serum Levels of Mineral Bone Metabolism and Inflammation Biomarkers in Patients with Stage 3a-5 Chronic Kidney Disease.

: Uric acid (UA) and the markers of mineral bone metabolism and inflammation are commonly altered in patients with chronic kidney disease (CKD) and are associated with the risk of cardiovascular complications and death. Studies point to a link between high serum UA and mineral bone homeostasis and inflammation, but controversy remains. The aim of this study was to evaluate the relationship between UA levels and mineral bone metabolism and inflammation biomarkers in a sample of Mexican patients with CKD 3a-5.

Memory T-Cells Contribute to Calcium Release from Bones during Lactation in Mice.

Milk production during lactation places a high demand for calcium that is fulfilled both from maternal bone resorption and diet. While it is known that mammary gland-derived PTHrP drives bone resorption during lactation, the impact of postpartum estrogen loss on bone has been unclear. We used a case-control study design to test the effect of estrogen loss in lactating mice.

VIPomas are a rare type of functional pancreatic neuroendocrine tumors (PNETs), causing symptoms due to their hypersecretion in the gastrointestinal tract. The rare association of PNETs with tumors of endocrine organs such as pituitary and parathyroid glands is called multiple endocrine neoplasia (MEN1) syndrome. Due to their indolent course, VIPomas often present late in the illness and may already have metastatic disease.

Article Synopsis
  • A 45-year-old woman with a long history of diffuse osteoarticular pain, especially in her lower back, was referred for a new imaging evaluation by a rheumatologist after having been diagnosed with spondyloarthritis 11 years prior.
  • She had been effectively treated for 3 years with a medication (adalimumab) before being lost to follow-up for 8 years and now shows worsening symptoms, including low back pain and fatigue.
  • Imaging studies including MRI and CT scans were ordered to investigate the underlying causes of her worsening pain, amidst complications like nonobstructing kidney stones and abnormal vitamin D levels.
