Ultrasonic endoscopy or endosonography is a diagnostic tool that currently becomes one of the most citated innovations to resolve difficult problems in gastroenterology. The article reviews the currently accepted indications by the American Society of Gastrointestinal Endoscopy.

Background And Aims: Factors predicting the need for step-up procedures after EUS-guided drainage (EUS-FCD) of peripancreatic fluid collections (PFCs) were explored in retrospective studies restricted to Walled-Off Necrosis (WON) and Lumen Apposing Metal Stents (LAMS).

Methods: All consecutive candidates for EUS-FCD between 2020-2024 were included in a Prospective Registry of Therapeutic EUS (PROTECT, NCT04813055), with prospective monthly follow-up evaluating clinical success, adverse events and recurrences. Prospectively assessed baseline clinical and morphological factors, including the Quadrant-Necrosis-Infection (QNI) classification, were included in a stepwise logistic regression model to predict the need for step-up.

Rationale: We report here a case of using iodine-125 (125I) seed implantation via endobronchial ultrasound (EBUS) in the treatment of malignant central airway obstruction (MCAO) in a patient with lung adenocarcinoma.

Patient Concerns: The patient still experienced MCAO after conventional bronchoscopic interventional therapy.

Diagnoses: The patient was diagnosed as lung adenocarcinoma stage IV (T4N2M1a).

Insulinoma is a pancreatic neuroendocrine tumour that primarily leads to episodes of hypoglycaemia due to inappropriate and excessive secretion of insulin. It classically presents with neuroglycopenic and autonomic sympathetic symptoms, which resolve promptly with glucose administration. Elevated level of insulin and Cpeptide in the presence of low plasma glucose level and absence of plasma sulfonylurea are diagnostic features.

Objectives: The objective is to develop and validate intratumoral and peritumoral ultrasomics models utilizing endoscopic ultrasonography (EUS) to predict pathological grading in pancreatic neuroendocrine tumors (PNETs).

Methods: Eighty-one patients, including 51 with grade 1 PNETs and 30 with grade 2/3 PNETs, were included in this retrospective study after confirmation through pathological examination. The patients were randomly allocated to the training or test group in a 6:4 ratio.
