Background: Statins, as an important class of lipid-lowering drugs, play a key role in the prevention and treatment of cardiovascular diseases. However, with their widespread use in clinical practice, some adverse events have gradually emerged. In particular, the hepatotoxicity associated with statins use has become one of the clinical concerns that require sufficient attention.

Aims: To compare adherence to perindopril/amlodipine/atorvastatin combination administrated as a polypill (one pill) vs separate tablets.
Methods: Using the healthcare utilization database of Lombardy (Italy), 1 110 patients who received the perindopril/amlodipine/atorvastatin polypill during 2019-2021 were matched with 1 110 patients prescribed the same combination in separate tablets or as two antihypertensive drugs in a single tablet and the lipid-lowering drug tablet separately. Adherence to treatment was assessed over the year after the first perindopril/amlodipine/atorvastatin dispensation as the proportion of the follow-up days covered by prescription (PDC).

Atorvastatin and fenofibrate are well-known lipid-lowering drugs. Atorvastatin acts by reducing the production of cholesterol through the inhibition of the 3-hydroxy-3-methyl-glutaryl-coenzyme A reductase (HMG Co-A reductase) enzyme, whereas fenofibrate is a PPAR-α agonist. Piperine is an alkaloid mostly found in black pepper fruits.

Background And Purpose: Statin therapy reduces the risk of ischemic stroke; however, certain studies have observed an increased incidence of intracerebral hemorrhage (ICH). Moreover, proprotein convertase subtilisin/kexin type 9(PCSK-9) inhibitors have emerged as a powerful class of lipid-lowering medications, potentially with a lower propensity for causing hemorrhagic events. To investigate this matter further, we conducted a network meta-analysis of randomized controlled trials (RCTs) involving statins and PCSK-9 inhibitors that reported occurrences of ICH.
