Glioblastoma Multiforme frequently metastasises from their original location by for example infiltration along white matter tracts [1]. GBM metastasis outside the central nervous system is distinctly rare though there are previous reports of spread to various organs [2-5]. We add an unusual case of a patient with aggressive cerebral GBM metastasis to the parotid gland and the lungs.

Glioblastoma multiforme (GBM) is a World Health Organisation (WHO) grade IV glioma originating from astrocytes. It is the most common malignant primary tumour of the brain and central nervous system (CNS) and is associated with fast progression and violent local spread, with a median overall survival of approximately 15 months after diagnosis. Due to its late and varied presentation, it is often diagnosed only after it has grown considerably.

Introduction: Extraneural metastases (ENM) from glioblastoma (GBM) remain extremely rare with only a scarce number of cases described in the literature. The lack of cases leads to no consensus on the optimal treatment and follow-up of these patients.

Research Question: Do patient or tumor characteristics describe risk factors for ENM in GBM patients, and is it possible to identify mechanisms of action?

Material And Methods: This study presents a 55-year-old man with diagnosed GBM who was referred to a CT due to reduced general condition and mild back pain which revealed extensive systemic metastases.

Background: Glioblastoma multiforme (GBM) is the most aggressive brain tumor malignancy in adults, accounting for nearly 50% of all gliomas. Current medications for GBM frequently lead to drug resistance.

Objectives: Umbelliferone (UMB) is found extensively in many plants and shows numerous pharmacological actions against inflammation, degenerative diseases and cancers.

Patients with metastatic pancreatic ductal adenocarcinoma survive longer if disease spreads to the lung but not the liver. Here we generated overlapping, multi-omic datasets to identify molecular and cellular features that distinguish patients whose disease develops liver metastasis (liver cohort) from those whose disease develops lung metastasis without liver metastases (lung cohort). Lung cohort patients survived longer than liver cohort patients, despite sharing the same tumor subtype.

Article Synopsis
  • GBM IDH wild type (GBM IDH wt) is linked to bad outcomes and intense inflammatory processes that help tumors grow and attract immune cells, making them more aggressive.
  • Researchers utilized RNA-seq and bioinformatics tools to explore how inflammatory molecules, specifically S100A proteins, play a role in glioma, finding a notable increase in S100A expression in GBM IDH wt compared to IDH mutants.
  • The study identified specific functions of S100A9, A11, and A13 in different regions of the glioma microenvironment, suggesting potential therapeutic strategies, such as using the RAGE inhibitor Azeliragon, currently in clinical trials, to counteract these inflammatory effects.
