Background: Patients with severe cerebral ischemia may lose autoregulation to increase cerebral blood flow following neural activity. Although the steal phenomenon under conventional cerebral blood flow study has been known as a high-risk factor for stroke, the cerebral oxygen hemodynamics in ischemic patients during functional activation has not been thoroughly investigated. In this study, we present rare cases with intracortical steal phenomenon during motor tasks detected by multichannel functional near-infrared spectroscopy before and after surgery.
Methods: The relative concentration change of oxygenated, deoxygenated and total hemoglobin in and around the primary sensorimotor cortex during contralateral hand grasping was investigated in 11 patients with severe internal carotid artery stenosis.
Results: In 3 patients, the concentration of total hemoglobin around the primary sensorimotor cortex significantly decreased in response to motor stimulation and returned to baseline soon after termination of the motor task. This phenomenon partially disappeared postoperatively in all patients who underwent surgery. The remaining 8 patients showed no signs of total hemoglobin decrease in and around the sensorimotor cortex. In 9 patients, lack of decrease in deoxygenated hemoglobin in the center of the primary motor cortex during the motor task was observed and 3 of them showed significant increase in deoxygenated hemoglobin.
Conclusions: We have demonstrated that in some patients with severe ischemia, an abnormal motor-related steal phenomenon can be observed. This phenomenon can be modulated by surgical intervention and might imply the severity of ischemia.

Background: The left internal thoracic artery (LITA) has been widely accepted as the standard for revascularizing the left anterior descending artery during coronary artery bypass grafting (CABG) surgery. However, in 10-20% of cases, the LITA may lead to unsecured side branches to the chest wall, particularly the lateral costal artery (LCA), potentially resulting in postoperative chest angina.
Case Presentation: We report the case of a 58-year-old patient who experienced persistent angina eight months after having undergone coronary artery bypass grafting (CABG) due to the steal phenomenon caused by a thick lateral costal artery (LCA).

Staged angioplasty (SAP) is used as an alternative to preventing cerebral hyperperfusion syndrome (CHS) after carotid artery stenting (CAS), particularly among patients with compromised cerebral haemodynamics. Although SAP is presumed to prevent CHS by gradually restoring cerebral blood flow (CBF), few studies have examined haemodynamic changes, and none have evaluated CBF during the chronic phase of SAP. In the present study, in addition to the clinical outcome of SAP, we measured both resting CBF and cerebrovascular reactivity (CVR) throughout all stages of SAP, including the chronic phase, using quantitative N-isopropyl-p-[I-123] iodoamphetamine (I-IMP) single-photon emission computed tomography (SPECT) and evaluated the effects of SAP on cerebral haemodynamics.

Micro and Small businesses (MSEs) face several challenges to survive in the market, some of these challenges are related to security matters which are the subject of this article. In Mexico MSEs are victims of crimes such as extortion, theft, and assaults, among others; this situation leads them to spend their economic resources to make structural changes to be able to continue with their commercial activities. This article is based on a survey conducted on 370 MSEs in the central region of the state of Veracruz in Mexico; the present study aims to evaluate the impact of structural changes made by businesspeople in their perception of business security.
