I describe a simple technique for performing capsulorhexis without viscoelastic material or expensive instruments. A slightly barbed, bent, 30-gauge needle is used to directly puncture clear cornea and create a capsulorhexis without the need for a groove or stab incision. Hydrodissection is carried out with the attached 1 cc syringe (tuberculin) filled with a balanced salt solution. Hydrodissection, hydrodelineation, and mobilization of the nucleus can be carried out before the eye is entered with a blade. With direct puncture, the technique is immune to the effects of high intraocular pressure and a shallow anterior chamber. The Technique is ideally suited for bimanual microincision phacoemulsification.

Background/objectives: Magnesium (Mg)-based food supplements contribute to the maintenance of adequate levels of Mg that are essential for overall health and well-being. The aim of this double-blind, randomized, cross-over clinical study was to assess the plasma Mg levels in volunteers following the oral administration of a magnesium-based nutraceutical ingredient, MAGSHAPE microcapsules (Mg-MS), in comparison to other commonly used magnesium sources, including the following: Mg Oxide (MgO), Mg Citrate (Mg-C), and Mg bisglycinate (Mg-BG).
Methods: A total of 40 healthy women and men were put on a low-Mg diet for 7 days, and after 8 h of fasting, a blood sample was taken from a digital puncture before (0 h) and 1 h, 4 h, and 6 h after the oral intake of each product.

Local anesthetic (LA)-induced peripheral nerve block (PNB) is an important part of multimodal analgesia to reduce postoperative pain, accelerate postoperative recovery, and improve clinical prognosis. The duration of LA depends on anesthetics, and the repeated nerve positioning, puncture injection or indwelling catheter is often required to prolong the effect of PNB. In this study, the genipin, was used to crosslink gelatin-based hydrogel, and then co-loaded with indocyanine green (ICG) and lidocaine as an LA-controlled release system (ICG@Lido/Gel and ICG@Lido/gGel).

Purpose: To illustrate a technique for the removal of subretinal gas via pars plana vitrectomy (PPV) with air-fluid exchange and simultaneous manipulation with scleral depression.
Methods: PPV to remove subretinal gas causing persistent macula-off retinal detachment was performed in one eye, and the results were evaluated in this case report. Ports were carefully placed to avoid puncturing the retina, which was significantly displaced anteriorly past the ora serrata due to the buoyancy of the subretinal gas with the patient in a supine position.
