A 65-year-old female presented to our hospital with a 6-month history of pollakuria, low-grade fever and urgent incontinence. Cystoscopy revealed a nonpapillary bladder tumor that was 50 mm in diameter in the trigon. Computed tomography showed the abscess between the urinary bladder and sigmoid colon. Transurethral resection was performed and the histology consisted of inflammatory lesions with inflammatory cell infiltration, which was diagnosed as an inflammatory pseudotumor due to diverticulitis of the sigmoid colon. Sigmoidectomy was subsequently performed. A fistula between the urinary bladder and the sigmoid colon was not detected. Cystoscopy 2 months after the operation revealed no signs of a bladder tumor.

Primary clear cell adenocarcinoma (CCA) of the urinary bladder is a rare and aggressive malignancy. Few reports in the literature describe this presentation, as associated with malignant transformation of endometriosis. This case highlights the complex etiology of this variant of CCA, initially diagnosed using comprehensive imaging and genetic analysis, and subsequently confirmed through extensive surgical intervention and chemotherapy.

Müllerianosis of the urinary bladder is a rare pathological condition involving ectopic Müllerian-derived tissues within the bladder. Despite its benign nature, due to its ability to mimic malignant conditions such as bladder carcinoma, diagnosis might be challenging. The indication for kidney transplantation over a urinary diversion (UD) for patients with severe lower urinary tract dysfunction and end-stage renal disease is a controversial issue.
