A 16-year-old girl was referred with a presumed muco-epidermoid carcinoma of the distal trachea, which was diagnosed by bronchoscopic biopsy. She underwent tracheal resection and end-to-end anastomosis. Final pathologic examination of the resected specimen revealed a benign oncocytic adenoma. This neoplasm is composed predominantly of oncocytes and is extremely rare in this location.

Oncocytoma is a rare benign neoplasm of the glandular tissue, most commonly found in the caruncle of the eye. These lesions have an excellent prognosis and have not been reported to recur after resection from the caruncle. Given the wide range of differentials and potential for malignancy, excision and histopathological examination are recommended to establish the diagnosis and guide proper treatment.

Oncocytic carcinoma is a rare malignant neoplasm of oncocytic cells, such cells being characterized by abundant granular eosinophilic cytoplasm, invasive growth, and pronounced cellular pleomorphism. It may arise de novo, or through malignant transformation of a pre-existing benign oncocytoma. Oncocytic carcinoma most commonly occurs in the salivary glands, with rare involvement of the ocular adnexa where it mainly affects caruncle, conjunctiva, and lacrimal sac, and only very arises in the lacrimal gland.
