AI Article Synopsis

  • The study focused on eight Chinese patients with hepatosplenic gammadelta T-cell lymphoma to explore their clinical and pathological features, as well as their immunophenotype and TCR gene rearrangements.
  • The patients exhibited enlarged spleens with histological infiltration by lymphoma cells, and immunohistochemical staining showed consistent expression of CD3, CD43, and CD56, with some cases also expressing CD8 and none expressing CD4.
  • Notably, the findings suggest that while the characteristics of this lymphoma type align with previous data, splenomegaly was less extreme in these patients, and a positive CD8 status was observed.

Article Abstract

Aim: To investigate the clinicopathologic characteristics, immunophenotype and TCR gene rearrangements of hepatosplenic T-cell lymphoma in eight Chinese patients.

Methods: Eight Chinese patients with hepatosplenic gammadelta T-cell lymphomas were studied. Hematoxylin-eosin-stained slides and clinical histories were reviewed. We also carried out immunohistochemical staining for CD3, CD4, CD8, CD20, CD43, CD56, CD79a, UCHL-1, and TCR gammadelta. Rearrangements of TCR gamma and delta chain genes were also studied.

Results: The spleens were enlarged and the cut surfaces were homogeneous and red-purple in color without identifiable gross lesions or enlarged hilar lymph nodes. Histologically, lymphoma cells infiltrated the cords of Billroth and often packed the sinuses. Liver biopsy showed lymphoma cell infiltrations in the sinusoids, and three cases showed involvements of the portal tracts. Immunohistochemically lymphoma cells were positive for CD3, CD43, and CD56 in all cases. Four of eight cases were positive for CD8, and all cases were negative for CD4 (6/6). Monoclonal rearrangements of TCR gamma gene were demonstrated by PCR analysis in five out of the eight cases. TCR delta gene rearrangements were detected in six out of the eight cases, which demonstrated single bands on PAGE gel, and the amplification products in two cases were confirmed by sequencing.

Conclusion: The clinicopathology of hepatosplenic gammadelta T-cell lymphoma in Chinese patients is similar to what was previously reported except that the splenomegaly is not so massive, and CD8 is positive.

Article Synopsis
  • T-cell leukemia from large granular lymphocytes (T-LGL leukemia) is a rare type of cancer, with γδT-LGL leukemia making up about 17% of cases and being linked to autoimmune diseases like rheumatoid arthritis (RA).
  • A study analyzed 15 patients with γδT-LGL leukemia and rheumatologic diseases, finding that most had RA, with notable symptoms such as splenomegaly and neutropenia.
  • The research highlighted that mutations were common, particularly in certain genes, and showed unusual characteristics of γδT-LGL leukemia, mainly affecting the spleen and not appearing in peripheral blood, prompting the need for further investigation into the relationship between this leukemia and RA.

Hepatosplenic Alpha-Beta T-Cell Lymphoma: A Challenging Diagnostic Entity.

Hepatosplenic T-cell lymphoma (HSTCL) is rare and clinically very aggressive T-cell lymphoma. The majority of cases harbor γδ T-cell receptors (TCRs); however, in some even rarer cases, tumor cells harbor αβ TCR. Recent studies suggest that αβ cases may have distinct morphological characteristics and demonstrate an even more aggressive course.

The patient was a 21-year-old man who had been diagnosed with Crohn's disease and received infliximab and azathioprine six years earlier. He was admitted with fever and fatigue. Peripheral blood examination showed LDH 2,473 U/l and thrombocytopenia, and contrast-enhanced computed tomography (CT) showed hepatosplenomegaly.
