Unlabelled: To report an unusual case of fungus keratitis due to Fonsecaea pedrosoi that developed after corneal trauma.

Case Report: A 18-year-old male presented with a corneal ulcer in the right eye, 28 days after a trauma with glass fragments. Corneal scrapings were collected for smears and culture. Dematiaceous hyphae were seen on wet mounts of the scrapings and dark pigmented colonies grew repetitively on the culture media; all colonies were identical, and were subsequently identified as Fonsecaea pedrosoi. Treatment was initiated with topical natamycin at one hour intervals, 200 mg oral ketoconazole per day and later changed to a combination of 200 mg ketoconazole and amphotericin B. In humid tropical regions Fonsecaea pedrosoi is one of the primary causes of human chronic cutaneous mycosis, chromoblastomycosis. Combination of systemic and topical antifungal medications may provide the best option for cure in corneal chromoblastomycosis.

Development of PCR-Multiplex Assays for Identification of the Herpotrichiellaceae Family and Agents Causing Chromoblastomycosis.

J Fungi (Basel)

August 2024

Laboratório de Micoses Superficiais e Sistêmicas, Seção de Bacteriologia e Micologia, Instituto Evandro Chagas, Ananindeua 67030-000, Brazil.

Article Synopsis
  • The Herpotrichiellaceae family consists of pathogenic fungi linked to diseases like chromoblastomycosis and phaeohyphomycosis, which present complex clinical symptoms and are found globally.
  • Identifying these fungi is difficult due to subjective morphological methods, leading researchers to rely on molecular techniques, particularly genetic sequencing of ribosomal DNA.
  • This study developed optimized PCR-Multiplex methodologies to improve the identification of these fungi, aiming to enhance infection management and provide more accurate epidemiological data.

Calcineurin activity in Fonsecaea pedrosoi: tacrolimus and cyclosporine A inhibited conidia growth, filamentation and showed synergism with itraconazole.

Braz J Microbiol

July 2024

Laboratório de Taxonomia, Bioquímica e Bioprospecção de Fungos (LTBBF), Instituto Oswaldo Cruz, Fundação Oswaldo Cruz, Rio de Janeiro, 21040-900, Brazil.

Article Synopsis
  • * The researchers discovered calcineurin activity in F. pedrosoi and found it can be inhibited by drugs like FK506 and CsA, which led to cell death and visible changes in the fungal structure.
  • * FK506 was the most effective treatment, with a lower required concentration to inhibit growth compared to CsA, and both drugs showed a synergistic effect when combined with itraconazole, highlighting calcineurin as a potential antifungal target.

Article Synopsis
  • - Chromoblastomycosis (CBM) is a chronic fungal disease primarily found in tropical areas, with limited data on its occurrence in French Guiana from 1950 to 2023, where a study identified 23 mostly male patients, mainly of Creole descent.
  • - The majority of cases presented lesions on the lower limbs and had a median diagnosis time of four years, with diagnostic methods including direct microscopy (78.3%) and mycological cultures (69.6%).
  • - Treatment approaches varied, involving antifungals like itraconazole and terbinafine, surgery, or a combination of these, with surgery recommended for new, localized lesions, highlighting differences in CBM compared to other mycoses

Article Synopsis
  • Chromoblastomycosis (CBM) and pheohyphomycosis (PHM) are fungal infections caused by dematiaceous fungi, with flucytosine (5-FC) historically used for treatment but often leading to resistance.
  • This study evaluated carmofur, a related drug, showing it has higher selectivity and potential synergistic effects when used with other antifungals against certain fungal strains.
  • While carmofur demonstrated some advantages in combating resistance, it also faced its own challenges, indicating the need for careful consideration in its clinical use alongside other treatments.

Identification of Chromoblastomycosis and Phaeohyphomycosis Agents through ITS-RFLP.

Chromoblastomycosis (CBM) and phaeohyphomycosis (FEO) are infections caused by melanized filamentous fungal agents, primarily found in tropical and subtropical regions. Both infections pose significant challenges for the correct identification of the causative agent due to their morphological similarity, making conventional methods of morphological analysis highly subjective. Therefore, molecular techniques are necessary for the precise determination of these species.
