Hypertrophic scar (HS) is a disease with excessive skin fibrosis and collagen disorder, which is generally caused by abnormal wound repair process after burn and trauma. Although intralesional injection of 5-fluorouracil (5-Fu) has been used in clinical treatment of HS, the patients' compliance of injection treatment is poor. In this study, a double-layer dissolution microneedle (MN) containing asiaticoside (AS) and 5-Fu was designed for the treatment of HS.

Intralesional injection of 5-fluorouracil for the clinical treatment of hypertrophic scars (HS) remains challenging due to its short half-life, as well as the absence of evidence-based dosage and frequency injection guidelines. Herein, we developed a matrix metalloproteinases (MMPs)/reactive oxygen species (ROS)-responsive injectable prodrug hydrogel (GFP) that exhibits sustained drug release and fluorescence imaging capability, aiming to facilitate the optimization of injection dosage and frequency in HS treatment. The GFP hydrogel comprises gelatin methacryloyl and pendant methacryloyl-decorated tetrapeptide (PPPK) with 5-fluorouracil acetic acid/rhodamine B at the N-terminus.

Management of Keloids and Hypertrophic Scars.

Keloid and hypertrophic scars are a result of aberrant wound healing responses within the reticular dermis. They are thought to be secondary to the formation of a disorganized extracellular matrix due to excessive fibroproliferative collagen response. Prevention of these scars focuses on avoiding elective or cosmetic procedures such as piercings in patients at high risk, reducing tension across the lesion, and decreasing the inflammatory response.
