We present a microangiogram study and clinical cases of super-thin flaps based on a transverse cervical perforator. This flap is the first to use the perforator of the superficial branch of the transverse cervical artery. This flap is more useful for providing colour and texture matches than a skin graft, and it is easier to harvest the flap than a free flap because it is a kind of skin flap.

Traumatic burst fractures of the atlas occur with axial loading of the cervical spine. Many of these injuries can be treated by nonsurgical management with external orthosis; however, cases with transverse ligament disruption or significant C1 lateral mass displacement require internal reduction and fixation. In patients with poor bone quality in the setting of osteoporosis or chronic illness, atlanto-axial fixation and reduction of the fracture can be a challenge, necessitating extension of fusion to the occiput, which significantly limits the range of motion.

Purpose: The main objective of this study was to conduct a radioanatomical study of the osteo-myo-cutaneous scapulo-dorsal pedicled flap.
Methods: A radiological study was performed to study the anatomical variations of the dorsal scapular pedicle (origin, course of the deep branch of the dorsal scapular artery (DSA) in relation to the medial border of the scapula, perforators from the superficial branch of the DSA). Perforators from the superficial branch of the DSA were also identified on anatomical subjects, and their cutaneous vascular territory was determined.
