Craniocervical tuberculosis (TB) is very rare. Despite the use of magnetic resonance imaging (MRI) and cranial tomography (CT), diagnosis of craniocervical tuberculosis is frequently difficult. In this study, we present a craniocervical tuberculosis abscess case which demonstrates the role of transoral surgery for both diagnosis and treatment.

Multiple cranial nerve palsies frequently accompany hypoglossal nerve palsy, potentially indicating malignancy, such as lymphoma, nasopharyngeal carcinoma, or metastases. However, when solely the hypoglossal nerve is affected, the causes may involve Chiari malformation, arachnoid cyst, or infectious mononucleosis, suggesting a positive prognosis. Craniocervical junction tuberculosis (TB), is an uncommon cause of isolated hypoglossal nerve palsy and has been reported infrequently in the literature.

Background: There are few guidelines on how to best manage craniovertebral junction (CVJ) tuberculosis (TB). Certainly, timely tissue diagnosis, immobilization of the neck, and decompression of CVJ with appropriate stabilization are the mainstays of treatment for TB at the CVJ.
