Objective: Surgical exposure of intrinsic lesions located lateral to the brainstem still represents a challenging task. The aim of this study was to assess the feasibility of the extracerebral far lateral supracerebellar infratentorial (FLSI) approach for the treatment of gliomas located in the upper brainstem in the pediatric population.
Methods And Results: Between 1992 and 2002, seven patients (mean age 8.7 years) with tumors of glial origin (WHO I-IV) located mainly in the pontomesencephalic region were operated with the FLSI approach in a sitting position. Satisfactory extent of resection without additional morbidity was achieved.
Conclusion: In a carefully selected pediatric patient population, the FLSI approach proved to be a feasible and effective surgical route for the treatment of upper brainstem gliomas.

A patient with reactivated varicella zoster virus (VZV) manifesting in the left-sided dermatome L3 and S2-S4 developed tonic spasms which morphed into myoclonic jerks, paresis, rigidity and hypoesthesia of the left leg. Later, stimuli-sensitive myoclonus progressed to affect the upper body and was accompanied by fever surges with high-frequency myoclonus, hypertensive derailment, dysphagia and other features of the brainstem with autonomic dysfunction. Cerebrospinal fluid tested positive for VZV, MRI showed no signs of myelitis and EEG was negative for epilepsy.

Introduction: Dynamic modulation of grip occurs mainly within the major structures of the brain stem, in parallel with cortical control. This basic, but fundamental level of the brain, is robust to ill-formed feedback and to be useful, it may not require all the perceptual information of feedback we are consciously aware. This makes it viable candidate for using peripheral nerve stimulation (PNS), a form of tactile feedback that conveys intensity and location information of touch well but does not currently reproduce other qualities of natural touch.

Extraventricular neurocytoma (EVN) is a rare neuronal tumor with a marked tendency towards ganglionic or glial differentiation. Although EVN commonly occurs in young adults, congenital cases are extremely rare, and standardized therapeutic strategies remain undetermined.
