Background: Carcinoma arising in a neovagina is rare.

Case: A patient with an adenocarcinoma arising from a neovagina constructed with use of a sigmoid segment.

Infertility is a condition affecting women who are born with an underdeveloped or absent vagina, a birth defect known as congenital absence of the vagina. It is a rare disorder where the development of the Mullerian duct is obstructed by unidentified causes. The case is seldom reported due to the low prevalence and sparse epidemiology studies worldwide.

Intestinal vaginoplasty, first described in 1904, has more recently become a popular mechanism for gender-affirming surgery in the United States. We present the case of a transgender female patient with retained foreign bodies in her neovagina, which required endoscopic therapy, provide a brief review of the literature, and discuss the potential long-term complications that can arise from a neovagina after intestinal transfer. It is important that gastroenterologists have awareness and recognition of these issues, as surgical reconstruction using intestinal segments for transgender patients becomes more common.

Despite strong hereditary components, most cases of colorectal cancer are sporadic. The possibility to manipulate in the clinical setting the many presumed risk factors is almost impossible, and long-term epidemiological studies are the only reliable form for comparisons. We performed a systematic review to analyze the reports of de-novo adenocarcinoma arising in the transposed colon, used for conduit after esophagectomy, after total gastrectomy, and for vaginal reconstruction.

Background: In view of the rarity of vaginal agenesis, malignancy arising in the neovagina is extremely rare.

Case Presentation: Here, we report a 76-year-old female with an adenocarcinoma arising in the sigmoid colon neovagina which was constructed 53 years ago for congenital vaginal agenesis. Vaginal endoscopy to examine vaginal bleeding revealed a protruding lesion occupying three quarters of the lumen in the vicinity of anastomosis of the residual vagina and sigmoid colon.
