There is a natural need to measure flow in normal as well as diseased coronary arteries since the reestablishment of normal flow is the aim of most coronary interventions. Miniaturization has enabled positioning both proximal and distal within the coronary anatomy and improved accuracy of Doppler velocity measurements. Measurement of coronary flow reserve adds functional data to coronary angiographic anatomic description, and quantitation of flow, achieved by combining Doppler velocity. Although new techniques involving transthoracic Doppler and magnetic resonance imaging are currently being developed and refined to measure coronary flow velocity and volume flow noninvasively, the role of intracoronary Doppler remains secure particularly for the coronary interventional physician.

Background: Epicardial stenosis and coronary microvascular dysfunction (CMD) may coexist in patients with chronic coronary syndrome (CCS). Microvascular resistance reserve (MRR) has been demonstrated to be a valid cross-modality metric using continuous saline infusion thermodilution and intracoronary Doppler flow velocity methods. Background: In patients with angina and non-obstructive coronary artery disease (ANOCA), diagnosis of coronary microvascular dysfunction (CMD) remains an unmet need. Magnetocardiography (MCG), is a rest-based, non-invasive scan that can detect weak electrophysiological changes that occur at the early phase of ischemia.

Currently, diagnosis and intervention in CAD are typically performed via minimally invasive cardiac catheterization procedures. Using current diagnostic technology, such as angiography and fractional flow reserve (FFR), interventional cardiologists must decide which patients require intervention and which can be deferred; 10% of patients with stable CAD are incorrectly deferred using current diagnostic best practices.

Background: Microvascular resistance reserve (MRR) is a recently introduced specific index of coronary microcirculation. MRR calculation can utilize parameters deriving from coronary flow reserve (CFR) assessment, provided that intracoronary pressure data are also available. The previously proposed pressure-bounded CFR (CFRpb) defines the possible CFR interval on the basis of resting and hyperemic pressure gradients in the epicardial vessel, however, its correlation to the Doppler wire measurement was reported to be rather poor without the correction for hydrostatic pressure.
