This article aims to: (1) describe the treatment of three consecutive completely edentulous patients with fixed implant-supported restorations, including simultaneous same-day immediate loading with fixed provisional restorations in both jaws; and (2) illustrate a step-by-step definitive restoration approach. All patients followed the same diagnostic protocol. One surgical intervention was performed for simultaneous implant placement in both maxilla and mandible. Following implant placement, all patients received a simultaneous, same-day, complete-arch, screw-retained provisional restoration in both jaws according to the "pickup" technique. After healing, final impressions were taken, and occlusion recorder devices were prepared on working casts. For the first occlusal record, the "half-provisionals" method was used. Definitive solid abutments were used for both laboratory and clinical procedures. After casting and finishing, the frameworks were tried in, and a new occlusal record was made. The definitive segmented restorations consisted of four fixed partial dentures in the maxilla and three in the mandible and were cemented with provisional cement. All patients received a nightguard. The pickup technique facilitated simultaneous immediate loading in both maxilla and mandible. A complete, fixed, segmented rehabilitation supported by six to eight anteroposterior implants is a reproducible treatment for completely edentulous patients when optimal anatomic conditions are present.

Edentulous patients with history of cleft palate and severe maxillary atrophy are challenging patients to rehabilitate through an oral approach. The difficulty of rehabilitating these patients lies in the unusual anatomy they present and the alteration of the vascular supply of local tissues caused by the previous performed surgeries, along with other causes. This report describes the successful dental rehabilitation with zygomatic implants of a patient with complete bilateral cleft palate who was edentulous and had severe maxillary atrophy.

The ultimate dental implant challenge is reconstruction of the severely resorbed maxilla, especially in patients with long-term edentulism or multiple previous failed attempts at dental implant placement and full-arch implant rehabilitation. Before implant dentistry, complete dentures, which are limited by poor retention and lack of support that the alveolar ridge offers in atrophic maxillae, were the only option for these patients. Pterygoid implants are now a valid and valuable resource for the rehabilitation of the posterior atrophic maxilla.

The present article describes a step-by-step maximally digitalized workflow protocol with computer-aided design and computer-aided manufacturing (CAD/CAM) in partial-arch edentulous patients rehabilitated with fixed dental prostheses and removable partial dentures (FDPs and RPDs). Facial digitalization, intraoral scans, and functional mandibular movement recordings were used to create a 4D virtual patient on commercially available CAD software. The fixed components including post-and-cores, both metal-ceramic with extra-coronal attachment and monolithic zirconia crowns, and the RPDs were manufactured by computer numerical controlled direct milling.
