Randomized Trial of Pulsatile and Nonpulsatile Flow in Cyanotic and Acyanotic Congenital Heart Surgery.

World J Pediatr Congenit Heart Surg

December 2024

Penn State Hershey Pediatric Cardiovascular Research Center, Penn State College of Medicine, Hershey, PA, USA.

Background: The study objective was to determine the impact of cardiopulmonary bypass perfusion modalities on cerebral hemodynamics and clinical outcomes in congenital cardiac surgery patients stratified by acyanotic versus cyanotic heart disease.

Methods: A total of 159 pediatric (age <18 years) cardiac surgery patients were prospectively randomized to pulsatile or nonpulsatile cardiopulmonary bypass and stratified by type of congenital heart disease: acyanotic versus cyanotic. Intraoperative cerebral gaseous microemboli counts and middle cerebral artery pulsatility index were assessed.

Visual detection of pulselessness by carotid artery sonography - A prospective observational study among medical students.

Resuscitation

December 2024

Department of Anesthesiology and Intensive Care Medicine, University Hospital of Giessen and Marburg, Campus Marburg, Marburg, Germany. Electronic address:

Aim: This cross-sectional study aimed to determine whether medical students with little to no ultrasound experience could correctly distinguish between 'pulsation present' and 'no pulsation present' after a short introductory video on the subject using ultrasound videos of the common carotid artery (CCA).

Methods: Ultrasound videos (B-mode, M-mode, and Color Doppler) of pulsatile (systolic blood pressure 70-80 mmHg) and non-pulsatile (cardiopulmonary bypass surgery, clamped aorta) CCA were created. These were demonstrated to the medical students for a period of ten seconds - corresponding to the duration of the manual pulse palpation during cardiopulmonary resuscitation (CPR).

In this study, it was aimed to assess the microstructural changes in the main central auditory pathway in cases with subjective tinnitus. In total, 101 subjects (52 cases with bilateral subjective non-pulsatile tinnitus and 49 healthy cases as the control group) were included in the study. Participants underwent pure tone audiogram and Diffusion Tensor Imaging-Magnetic Resonance Imaging (DTI-MRI) examination with a 3 Tesla MRI device.

Article Synopsis
  • - This study assessed how well different types of arterial and venous cannulae perform in a pediatric ECMO setup under both pulsatile and non-pulsatile flow conditions.
  • - The researchers found that in pulsatile flow, lower pump speeds were needed, and the flow increase in the mock circuit was less significant, while non-pulsatile conditions resulted in higher pressures and overall energy levels.
  • - The conclusion suggested that non-pulsatile conditions offer better support with higher total hemodynamic energy, but pulsatile conditions are more physiological and are recommended for testing ECMO systems.

Is Transcutaneous Electrical Nerve Stimulation Therapy the Answer for Tinnitus: A Prospective Study.

Indian J Otolaryngol Head Neck Surg

October 2024

Department of Otorhinolaryngology and Head & Neck Surgery, Government Medical College, Patiala, Punjab India.

The study was aimed at determining the effectiveness of transcutaneous electrical nerve stimulation in treatment of tinnitus using Tinnitus handicap inventory (THI) and Visual analogue scale (VAS). 60 patients with bilateral, non-pulsatile, chronic subjective tinnitus; aged 18-65 years were subjected to 8 sessions (2 sessions/week) of TENS therapy given over a total period of 4 weeks. Pre-treatment and Post-treatment VAS, THI scores were compiled and analyzed statistically.
