Traumatic ankle conditions can lead to long-term sequelae if a pathologic process is misdiagnosed. The clinical presentation of an osteochondral lesion of the talar dome requires the clinician to have a high index of suspicion, and advanced imaging is often necessary to make the final diagnosis. Treatment should be initiated once the lesion is appropriately staged by radiologic or magnetic resonance imaging. We discuss the use of arthroscopy-assisted retrograde drilling of the medial talar dome that spares the articular cartilage within the talotibial articulation.

Background: Supination external rotation (SER) fibula fractures with abnormal medial clear space (MCS) on stress radiographs that normalizes with weightbearing radiographs, termed SER4a ankle fractures, pose a challenge in treatment decision making. This study aims to quantify differences in joint contact area, tibiotalar rotation, and translation using weightbearing computed tomography (CT) scans in SER4a ankle fractures.
Methods: Between November 2022 and September 2023, patients presenting with isolated Weber B fibula fractures were evaluated for inclusion.

Osteochondral lesions of the talus are chondral lesions affecting the subchondral bone mostly due to acute ankle trauma, including either sprains or fractures. After failure of conservative treatment, operative treatment is necessary, with different surgical techniques described in the literature. We describe a single-step osteochondral autograft transfer to access the medial talar dome lesion that avoids the need for a medial malleolar osteotomy and therefore eliminates morbidity while reducing operative time.

Background: The etiology of medial-sided talar osteochondral lesions (OCLs) remains insufficiently understood.
Purpose: To identify anatomical risk factors contributing to the development of unilateral or bilateral OCL of the talus on the medial side, utilizing morphological parameters derived from magnetic resonance imaging (MRI).
Material And Methods: In this retrospective study, 24 ankle MRI scans from 12 patients exhibiting bilateral OCLs of the talar dome on the medial side, 24 ankle MRIs from 24 patients with unilateral medial-sided OCLs, and 24 healthy controls matched for age, sex, and side within each group were analyzed.

Biological regulation of microfracture has been suggested to improve the quality of cartilage repair in patients.
