Anti-nuclear mitotic apparatus (NuMA) 1 antibodies are uncommonly detected in routine antinuclear antibody (ANA) screening. We present the case of a 65-year-old female with rheumatoid arthritis undergoing golimumab biotherapy who developed lupus-like symptoms including photosensitivity, fatigue, weakness, myalgias, alopecia, oral ulcers, and worsening of arthritis. Elevated serum levels of NuMA-1 antibodies were detected using indirect immunofluorescence (IIF) on HEp-2 cells with a titer of 1:1000, but no other ANA patterns were associated.

Aim: We have previously reported the mid-term efficacy and safety of tacrolimus (Tac)-based immunosuppressive therapy in such patients, and herein, we aimed to determine their long-term outcomes (over 10 years).
Methods: We retrospectively evaluate the data of 13 consecutive patients with biopsy-proven long-standing LN who underwent a long-term Tac-based treatment regimen. Tac was administered once daily at a dose of 3 mg as reinduction or maintenance treatment.

Backgrounds: Progranulin (PGRN) is a growth factor-like molecule with diverse roles in homeostatic and pathogenic processes including the control of immune and inflammatory responses. Some antinuclear antibodies (ANAs) bind extracellular nucleic acids released into tumor environments and are pulled into the nuclei of live cancer cells through nucleoside salvage pathways, independent of tumor-specific surface antigens. Here we show that ANA nuclear penetration induces nuclear flux by the lysosomal protease cathepsin B and leverage this mechanism to design an antinuclear antibody-drug conjugate (ANADC) with cathepsin B-labile drug linker. The ANADC targets nucleic acid exhaust from necrotic tumors and crosses membrane barriers through nucleoside salvage as a DNA-seeking and tumor agnostic "antinuclear missile" cancer therapy.
