Hyperuricemia developed in 2 children with autoimmune hemolytic anemia with reticulocytopenia at a time of hemolytic crisis. One likely cause of hyperuricemia is the destruction of nucleated RBC precursors by autoantibodies. It is advised that patients with autoimmune hemolytic anemia with reticulocytopenia be examined for hyperuricemia. This might explain the reason for reticulocytopenia and might prevent unnecessary bone marrow procedures. When hyperuricemia is present, supportive therapy might be needed to prevent renal damage.

Autoimmune enteropathy (AIE) is a rare cause of chronic diarrhea associated with autoantibodies and susceptibility to other autoimmune diseases, such as rheumatoid arthritis, diabetes, autoimmune hemolytic anemia, and atopic dermatitis. While it is more common in children, the prevalence of AIE in adults is increasing. Due to the nonspecific nature of its presenting symptoms and the lack of consistent findings, AIE can be challenging to diagnose.

Post-streptococcal autoimmune neuropsychiatric disorders (PANDAS) are a group of pathological condition characterized by sudden-onset obsessive-compulsive and tic disorders following beta-hemolytic Streptococcus group A (GAS) infection, hypothesized to be caused by autoimmune mechanisms targeting the basal ganglia. Scant literature is available regarding the microbiota composition in children with PANDAS, however few studies support the hypothesis that streptococcal infections may alter gut microbiota composition in these patients, leading to chronic inflammation that may impact the brain function and behavior. Notable changes include reduced microbial diversity and shifts in bacterial populations, which affect metabolic functions crucial for neuroinflammation.

There are two reported cases of autoimmune hemolytic anemia as a paraneoplastic presentation of gastric duplication in the adult population, but there have not yet been any reported cases of this in childhood. We report the first case in the pediatric patient population: the case of GDC presenting as autoimmune hemolytic anemia in a 15-month-old girl.
