Purpose: Levetiracetam (LEV) is a new compound with anticonvulsive efficacy in focal and generalized epilepsies. Recent in vitro studies suggest LEV to act as a selective N-type-calcium-channel blocker.

Methods: We used transcranial magnetic stimulation (TMS) in order to investigate if ion-channel blockade is relevant to the inhibitory CNS effects of LEV in vivo and if motor thresholds (MTs) are a valid TMS parameter to detect this mode of action. In a double blind, placebo-controlled, crossover study, the effects of single oral doses of 500 and 2000 mg LEV on motor thresholds, recruitment curves (REC), cortical induced silent period (CSP) and on intracortical inhibition (ICI) and facilitation (ICF) were studied in 10 healthy subjects.

Results: A significant increase of motor thresholds was noticed after 2000 mg LEV as compared to placebo. The recruitment curve showed a trend towards motor evoked potential (MEP) amplitude reduction after LEV. LEV had no significant effect on CSP or on intracortical excitability as measured by inhibition and facilitation.

Conclusions: We conclude that the modulation of ion-channel function, reflected by motor threshold elevation and a trend towards recruitment curve suppression, is relevant to the inhibitory CNS effects of LEV in vivo, and therefore, may contribute to the anticonvulsive efficacy of LEV. GABAergic or glutamatergic mechanisms seem to be less important in vivo as measured by TMS.

Sensory and Motor Function, Pain, and Health Status in Children with Arthrogryposis and Myelomeningocele.

Children (Basel)

December 2024

Department of Women's and Children's Health, Division of Paediatric Neurology, Karolinska Institutet, Karolinska University Hospital, S-17176 Stockholm, Sweden.

Background/objectives: Proprioception and sensory disorders have been reported in children with arthrogryposis multiplex congenita (AMC) and myelomeningocele (MMC), but valid and reliable assessment tools are limited in accurately identifying the sensory aspects of motor disorders. This study aimed to investigate the somatosensory status in the feet and legs. An additional purpose of this study was to explore pain, skin irritations, and health status.

Background: Intraneural edema is an important factor in the pathophysiology of carpal tunnel syndrome (CTS). Manual Lymphatic Drainage (MLD) is a manual treatment widely used to treat edema in a variety of conditions.

Purpose: This study aimed to evaluate the effect of MLD on intraneural edema of the median nerve in CTS patients, as well as its impact on symptom severity and hand function.

Background: There is growing interest in use of transcutaneous spinal stimulation (TSS) for people with neurologic conditions both to augment volitional control (by facilitating motoneuron excitability), and to decrease spasticity (by activating inhibitory networks). Various electrode montages are used during TSS, with little understanding of how electrode position influences spinal circuit activation. We sought to identify the thoracolumbar electrode montage associated with the most robust activation of spinal circuits by comparing posterior root-muscle reflexes (PRM reflexes) elicited by 6 montages.

Article Synopsis
  • The study aimed to assess the sensorimotor function in patients with unilateral chronic ankle instability (CAI) and compare their abilities to healthy controls, focusing on sensory reweighting and vestibular modulation.
  • Twenty individuals with unilateral CAI and twenty healthy participants underwent various tests to evaluate ankle proprioception, plantar sensation, balance, and motor control, using a mix of analyses to compare the groups.
  • Results indicated that those with CAI had increased sensory thresholds for light touch and two-point discrimination bilaterally, moderate deficits in certain motor control measures, and showed postural instability in balance tests when compared to healthy controls.

Effects of Smart Goggles Used at Bedtime on Objectively Measured Sleep and Self-Reported Anxiety, Stress, and Relaxation: Pre-Post Pilot Study.

JMIR Form Res

January 2025

Scientific Research, SleepScore Labs, 2175 Salk Avenue, Suite 150, Carlsbad, CA, 92008, United States, 1 858-299-8995.

Background: Insufficient sleep is a problem affecting millions. Poor sleep can trigger or worsen anxiety; conversely, anxiety can lead to or exacerbate poor sleep. Advances in innovative consumer products designed to promote relaxation and support healthy sleep are emerging, and their effectiveness can be evaluated accurately using sleep measurement technologies in the home environment.
