Purpose: This study introduces the Cloverleaf Internal Limiting Membrane (ILM) flap technique for managing challenging macular holes, aiming to enhance ILM flap placement and improve functional and anatomical outcomes.
Methods: A retrospective review of cases undergoing the Cloverleaf ILM flap technique was conducted at a single center. This technique involves creating multiple ILM flaps in a cloverleaf configuration in order to enhance the stability and coverage of the macular hole.

Background: In patients with Fuchs endothelial corneal dystrophy (FECD), the most beneficial stage to perform Descemet membrane endothelial keratoplasty (DMEK) remains uncertain. The goal of this study was to compare the surgical outcomes after DMEK in FECD patients with subclinical corneal edema and clinical corneal edema to test the hypothesis of whether performing surgery in subclinical corneal edema stages achieves better surgical outcomes.
Methods: In this prospective, observational, single-institution cohort study, 106 pseudophakic eyes of 85 patients with FECD were divided into two groups depending on the presence of preoperative subclinical and clinical corneal edema.

Objective: To describe long-term outcomes of posterior lamellar [Descemet stripping automated endothelial keratoplasty (DSAEK) and non-Descemet stripping (n-DSAEK)] and the role of microscope-integrated optical coherence tomography (Mi-OCT) in buphthalmic eyes.
Design: Retrospective study.
Methods: The medical records of all patients who were diagnosed with buphthalmos and underwent Mi-OCT-guided standard DSAEK/n-DSAEK since 2015 were evaluated.
Purpose: To describe a series of six cases in which iatrogenic macular holes were induced by the infusion fluid flow of a 25-gauge pars plana vitrectomy (PPV) system.
Methods: This is a retrospective case series of six patients treated at Kresge Eye Institute between 2018 and 2024.
Results: Iatrogenic macular holes caused by infusion fluid flow are a rare complication of PPV.

Objective: Demonstrate the variability of the spherical and toric ratios between the refraction at the intraocular lens (IOL) plane and the refraction at the spectacle plane when the sphere and the cylinder power of toric piggyback IOLs are calculated. Also, a comparison of the results of piggyback IOL power calculation with variable ratios, to the IOL power calculation performed by a major manufacturer online calculator using a fixed ratio.
