Objective: To introduce a new surgical approach, oophoropexy to prevent recurrent adnexal torsion.
Design: Case report and review of the literature.
Setting: The obstetrics and gynecology department of a university hospital.
Patient(s): A 29-year-old pregnant patient who had three events of torsion of the adnexa after stimulation.
Intervention(s): Laparoscopic oophoropexy.
Main Outcome Measure(s): Incidence of torsion.
Result(s): No more torsion events were registered during the ongoing pregnancy.
Conclusion(s): Laparoscopic oophoropexy is recommended in emergency situations to increase adnexal salvage and to prevent a recurrence.

Hypermobile Ehlers-Danlos syndrome (hEDS) is the most common type of EDS, characterized by joint hypermobility, frequent dislocations, and chronic pain. Genetic markers are not typically used in diagnosis. Massive ovarian edema (MOE) is a rare benign condition presenting as unilateral ovarian enlargement with stromal edema, and only a limited number of MOE cases during pregnancy have been reported. MOE is often complicated by ovarian torsion, which requires detorsion. Although the diagnosis of MOE can be made using ultrasound and magnetic resonance imaging, its rarity makes diagnosis difficult, usually leading to overtreatment.
