The management of splenic trauma is determined by factors related to a patient's clinical state. Currently, non-surgical management is favoured because of morbidity related to laparotomy and splenectomy. However, the presence of a pseudoaneurysm might necessitate selective splenic arteriography and embolization. Even though this also has associated morbidity, conservation of viable splenic tissue is a desired result.

Splenic infarction with infectious mononucleosis (IM) caused by Epstein-Barr Virus (EBV) has been reported as a rare complication of IM. The monospot test, often used to diagnose EBV-related IM, may produce false-negative results, especially in atypical presentations or early stages of infection. This report describes the case of a monospot-negative patient who developed splenic infarction as a complication of IM.

Acid sphingomyelinase deficiency (ASMD) is a rare, progressive lysosomal storage disorder resulting from a deficiency in acid sphingomyelinase, leading to sphingomyelin accumulation and multi-organ damage. ASMD presents a broad phenotypic spectrum with a continuum of severity, making it challenging to predict the phenotype in very young children and differentiate between acute and chronic neurovisceral disease.
