Toward a definition of colonic inertia. | LitMetric

Toward a definition of colonic inertia.

World J Gastroenterol

Department of Clinical and Experimental Medicine, University of Perugia Medical School, Italy.

Published: September 2004

Chronic constipation is a relatively frequent symptom; among its subtypes, the so called-colonic inertia represents a disease condition that is often considered for surgery. However, to date, there has been no agreement on definition of colonic inertia, and a literature review showed that this definition was given to numerous entities that differ from each other. In this paper these concepts are reviewed and a more stringent definition of colonic inertia is proposed.

Background: Total esophageal reconstruction after failure of conventional techniques is a significant surgical challenge, particularly in fragile, multi-operated patients. Traditional options, including gastroplasties, colon transfers, and free jejunal flaps, carry high morbidity, failure rates, or surgical complexity.

Material And Methods: We report an innovative four-step strategy for esophageal reconstruction using a presternal skin tube created with tissue expanders and perforator flap techniques.

Introduction: The association between post-polypectomy bleeding (PPB) and thrombocytopenia remains unclear, and current evidence is insufficient to provide definitive guidelines for managing patients with thrombocytopenia undergoing endoscopic procedures. We assessed the association between thrombocytopenia and PPB to identify the minimum platelet count threshold for safe colonoscopic polypectomy.

Methods: This large cohort study included patients who had undergone colonoscopy and polypectomy in Korea between 2005 and 2022.

Patients with long-standing inflammatory bowel disease (IBD) involving the colon are at higher risk of developing colorectal dysplastic or neoplastic lesions. While from sporadic colorectal cancer follows an "adenoma-carcinoma" sequence, IBD colitis-associated carcinogenesis is mainly related to an "inflammation-dysplasia-carcinoma" sequence. Currently, specific endoscopic surveillance strategies involving dye spray and virtual chromoendoscopy have been standardized, aiming for early CRC diagnosis.

Purpose: Proctocolectomy with ileal pouch-anal anastomosis (IPAA) is the definitive surgical treatment for patients with ulcerative colitis or familial adenomatous polyposis. While laparoscopic surgery has been widely adopted, robotic surgery has emerged as a promising alternative, particularly for complex pelvic dissection. However, the robotic learning curve presents significant challenges.
