Background: To establish a culture of safety and improve patient care, the Veterans Health Administration (VHA) is identifying and implementing necessary parameters and objectives across the health care landscape to enhance services on its journey to becoming a high reliability organization (HRO).
Methods: This quality improvement initiative sought to increase the understanding of factors that influence the establishment and sustainment of a just culture and identify specific methods for improving their implementation. Focus groups of HRO leads at 16 VHA hospital facilities identified emergent themes, facilitators, and barriers to maintaining a just culture and developed recommendations for enhancing both psychological safety and accountabilitity.

Background: Blood culture contaminants can lead to inappropriate antibiotic use, prolonged length of stay, and additional hospital costs. Several devices have been developed to reduce the risk of blood culture contamination by diverting a portion of the initial blood sample from the blood culture bottle. During multi-agent combat simulation exercises, accurately assessing the quality of collected combat data is a critical step. Addressing the current issue of low accuracy in combat data quality evaluation, which fails to effectively support simulation exercises, this paper proposes a Double-Weighted FAHP optimized by CVF (comparative value function) method for assessing combat data quality. First, a three-tiered evaluation framework for combat data quality indicators is established, with threshold values determined for each indicator.
