Objectives: To report the 7th case of pararenal angiomyolipoma published in the world literature and to review the international bibliography.

Methods: We report the case of a 46-year-old female with history of renal colic and a complex mass on radiological tests.

Results/conclusions: Extrarenal retroperitoneal angiomyolipoma is a rare pathology with no more than 7 published cases. The diagnostic difficulty and radiological similarities with liposarcoma make surgery the treatment of choice.

Angiomyolipomas are benign mesenchymal lesions often diagnosed incidentally, composed of adipose tissue, dysmorphic blood vessels, and smooth muscle. They are usually unilateral and symptomatic only when larger than 4 centimeters, posing a risk of spontaneous rupture and severe retroperitoneal hemorrhage. Treatment varies from conservative management to selective embolization or nephrectomy, depending on tumor size and patient condition.

With the continuous development of robot-assisted technology, Robot-assisted Laparoscopic Partial Nephrectomy (RALPN) has gradually become an optional method for the treatment of Hemorrhage secondary to angiomyolipoma (HSA). However, there are rare clinical reports of the primary RALPN for HSA. Therefore, this research aims to evaluate the efficacy and safety of primary RALPN for HSA.
