I describe a technique for surgical restoration of opposition of the fifth finger, which includes transfer of the extensor digiti quinti tendon between the fifth and fourth metacarpals towards the dorsoulnar aspect of the neck of the fifth metacarpal, where the transferred tendon is fixed to the opponens digiti quinti tendon. I have operated on 11 patients with paralysis of all the hypothenar muscles, all of whom were followed up from one to three years postoperatively. Ten were successful.

Parkinson's disease (PD) is a neurodegenerative disorder that predominantly affects dopaminergic neurons in the substantia nigra and ventral tegmental area, resulting in symptoms such as tremors, muscle rigidity, bradykinesia, and potential cognitive and affective disturbances. The effective delivery of pharmacological agents to the central nervous system is hindered by various factors, including the restrictive properties of the blood‒brain barrier and blood‒spinal cord barrier, as well as the physicochemical characteristics of the drugs. Traditional drug delivery methods may not provide the therapeutic concentrations necessary for functional restoration in PD patients.

Background/objective: Stiff person syndrome (SPS) and type 1 diabetes (T1D) are heterogeneous disorders characterized by antibodies (Abs) against glutamic acid decarboxylase (GAD).

Case Report: We describe 2 patients with T1D and autoimmune thyroid disease who presented with muscle rigidity and intermittent spasms that affected gait and with elevated circulating anti-GAD titers. Classic SPS and stiff limb syndrome were diagnosed, respectively.
