Previous clinical reports have shown that capecitabine, an oral prodrug of 5-fluorouracil (5-Fu), can induce peripheral neuropathy, resulting in numbness, paresthesia and hypoesthesia. However, the mechanism through which capecitabine causes peripheral nerve injury remains unclear. Here, we demonstrate that systemic administration of capecitabine leads to myelin abnormalities in the peripheral nerves of mice, which are possibly attributed to the death of Schwann cells, the myelinating cells in the peripheral nervous system.

Introduction: The medication 5-fluorouracil is known to cause cardiotoxic effects (with an incidence ranging from 5% to 18%), such as rhythm abnormalities and cardiomyopathies, including takotsubo cardiomyopathy. Capecitabine, an oral prodrug of 5-fluorouracil, has rarely been reported to cause cardiotoxic effects compared with its parent drug.

Case Presentation: An 80-year-old woman presented to the hospital with chest pain after recent initiation of capecitabine use for anal cancer.

Background: Cancer patients receiving chemotherapy often complain of "chemobrain" or cognitive impairment, but mechanisms remain elusive.

Methods: A patient with gastric cancer developed delirium and hyperammonemia after chemotherapy with the 5-fluorouracil pro-drug capecitabine. Exome sequencing facilitated a search for mutations among 43 genes associated with hyperammonemia and affecting the urea cycle directly or indirectly.
