Article Abstract

Objective: To determine whether glutamat and aspartat enriched cold crystalloid cardioplegia which was given in antegrade way has any effect on the myocardial protection during cardiopulmonary bypass.

Methods: Thirty-four patients who were electively undergone open heart surgery at Osmangazi University Faculty of Medicine, thoracic and cardiovascular surgery department, between March 2001 and May 2001 were included in this study. The patients were divided in two groups, each consisting of 17 patients. In group 1 coronary artery bypass surgery (CABG) was performed in 11 patients, mitral valve replacement (MVR) in 3 patients, aortic valve replacement (AVR) in 1 patient and AVR and MVR in 2 patients. While in group 2 CABG was performed in 13 patients and MVR was done in 4 patients. Group 1 patients received antegrade glutamat and aspartat (15 mmol/L) enriched cold crystalloid cardioplegia and group 2 patients were given cold crystalloid cardioplegia by antegrade route. Age, gender, diabetes mellitus, hypertension, preoperative myocardial infarction, smoking, ejection fraction, aortic cross-clamp time, need to defibrillation, inotropic support, and intraaortic balloon pump were recorded. The levels of cardiac troponin I (cTI) and creatine kinase myocardial band fraction (CK-MB) were measured in arterial blood samples at five different times. Statistical analysis was performed using Student's t-test and Chi-square test.

Results: There were no statistically significant differences in cTI and CK-MB values in blood samples taken at 5 different times pre and postoperatively between group 1 and group 2.

Conclusion: It is concluded that glutamat and aspartat enriched cold crystalloid cardioplegia does not have any effect on myocardial protection.

Very high heat is generated during the polymerization of poly (methyl methacrylate) (PMMA) bone cement, which is used for implant fixation in orthopedic surgery. As such, it has been suggested that irrigating the bone cement layer in the surgical site with a saline solution is a way of cooling the layer. In this study, we aimed to determine the influence of irrigation with a saline solution on the flexural strength and the microstructure of the test specimens of two PMMA bone cement brands: Simplex P and FIX 1.

Objective: To assess the impact of mild hypothermia on the distribution and elimination of an IV crystalloid fluid bolus in healthy anesthetized cats using volume kinetic (VK) analysis.

Methods: 10 adult cats were anesthetized and included in a prospective, randomized, cross-over study. The subjects were maintained either normothermic (38.

There have been few recent innovations since the introduction of cardioplegia more than 50 years ago. Surprisingly, cardioplegia as one of the most essential steps in terms of heart muscle protection during a surgical procedure requiring cardiac arrest has never been really standardized. As a consequence, a considerable variety of cardioplegic solutions and applications have developed: cold versus warm, crystalloid versus blood cardioplegia, antegrade versus retrograde or both, as well as different time schedules for repeated administration.

Introduction: Cold saline application through an irrigation catheter may induce reversible inhibition of submyocardial excitation, which may be used to identify in advance an ideal site for radiofrequency (RF) energy delivery around delicate areas.

Methods: An open irrigation catheter was positioned vertically or parallel with 10-g contact on coronary perfusing porcine hearts and the contacted myocardium was cooled by cold saline at 4°C (20 mL/min). A temperature electrode was inserted 2 mm below the myocardial surface (intra-myocardial temperature at approximately 2 mm below the surface [Temp-BS]).

Redo extended thoracic aortic replacement from aortic root to descending aorta via anterolateral thoracotomy with partial sternotomy for graft infection.

Division of Cardiovascular Surgery, Department of Surgery, Kobe University Graduate School of Medicine, 7-5-2 Kusunoki-cho, chuo-ku, Kobe, Hyogo, 650-0017, Japan.

A 59-year-old male underwent Bio-Bentall + total arch replacement with a frozen elephant trunk for acute type A aortic dissection before at another hospital. He was diagnosed as mediastinitis and previous graft infection, followed by wound closure with omental flap installation. However, the recurrent graft infection from the aortic root to the FET in the descending aorta was diagnosed by 18-fluorodeoxyglucose positron emission tomography.
