Objective: To document the vascular anatomy of the distally based superficial sural artery flap and to study the vascular anastomoses between the superficial sural artery and the septocutaneous perforators of the peroneal artery.

Methods: Ten fresh human cadavers were injected with lead oxide, gelatin and water. Twenty lags were then dissected and an overall map of the cutaneous vasculature was constructed. Vascular communications between the superficial sural artery and the lowest septocutaneous perforator of the peroneal artery was evaluated to determine the cutaneous vascular territory of the superficial sural flap. The distally based superficial sural artery island flap was used in 26 cases.

Results: There is constant vascular anastomosis between the superficial sural artery and the lowest septocutaneous perforator of the peroneal artery. The 26 flaps survived uneventfully except for two of partial fat necrosis.

Conclusion: The anatomic information enhances our understanding of flap design.

Purpose: Acute midsubstance Achilles tendon ruptures are the most common tendon injuries in adults and the choice of surgical technique is still debated. The objective of this study is to evaluate the rate of iterative ruptures and postoperative complications at a minimum follow-up of one year of percutaneous sutures of the Achilles tendon, as well as to investigate the risk factors for rupture.

Methods: In this single-center retrospective study, 142 patients (119 men and 23 women, mean age of 46 years) were reviewed at a mean follow-up of 24 (12-38) months.

Article Synopsis
  • Hemodialysis-related restless legs syndrome (HD-RLS) is a common issue in patients undergoing hemodialysis, diagnosed primarily through clinical criteria, with proposed additional use of nerve physiology measurement as a diagnostic aid.
  • The study involved 20 patients with HD-RLS, 33 without RLS (HD-nRLS), and 30 healthy controls, focusing on nerve conduction assessments in various limbs.
  • Results indicated that HD-RLS patients exhibited more severe nerve damage and longer F-wave durations in their nerves, suggesting that the F-wave duration to compound muscle action potential duration ratio could help in diagnosing HD-RLS.

Introduction: Medial femoral cutaneous (MFC) sensory nerve action potentials (SNAPs) can be easily recorded using distal stimulation. This study aimed to identify a new parameter using MFC SNAPs for the early electrophysiological diagnosis of length-dependent axonal polyneuropathy (LDAP) associated with uremic neuropathy.

Methods: Patients with chronic renal failure who were referred to the electrodiagnostic laboratory due to symptoms suggesting polyneuropathy were included.

Article Synopsis
  • - The study explores how leg dominance affects the choice of perforator flap donor sites for reconstruction surgeries using advanced imaging techniques to enhance preoperative planning.
  • - In a sample of 40 patients, most (87.5%) were right-leg dominant, showcasing that the dominant leg typically has larger and more numerous perforators for certain flap types compared to the nondominant leg.
  • - Results suggest that surgeons may benefit from prioritizing the dominant leg as a donor site, as it has better vascular characteristics and muscle thickness for improving surgical outcomes, although no significant differences were found in the subcutaneous tissue thickness.
