Background: Clarity and accuracy of the pharmacy aspects of cancer clinical trial protocols is essential. Inconsistencies and ambiguities in such protocols have the potential to delay research and jeopardise both patient safety and collection of credible data. The Chemotherapy and Pharmacy Advisory Service was established by the UK National Cancer Research Network, currently known as National Institute for Health Research Clinical Research Network, to improve the quality of pharmacy-related content in cancer clinical research protocols.

Philip Darbyshire has apparently gained the impression that I used 'gee-whiz' social psychology to 'absolve poor or negligent practice from any hint of personal responsibility and accountability'. In this reply, I first discuss the fundamental attribution error, which Darbyshire trivialises as the 'fundamental arrogance error' and a piece of 'linguistic puffery'. Second I examine Darbyshire's rather naive view of causation.

John Cree Wilson, Sr, was born in Santa Ana, California, and received his undergraduate degree from the University of California in 1908 and medical degree from the University of California, San Francisco, in 1912 [2]. He had a one year internship, then entered private practice, but apparently believed he needed more training and in 1916 quit his private practice for postgraduate training at the Massachusetts General Hospital. He completed that training, then served in military hospital at Fort McPherson, Georgia, when the US entered WW I.
