The brain has long been viewed as an insulin-insensitive organ. Following the demonstration of insulin receptors in the brain, this assumption has been challenged, and a whole new field of research has emerged. Insulin appears to play a role in brain physiology, and disturbances of cerebral insulin signalling and glucose homeostasis are implicated in brain pathology. This special issue of the European Journal of Pharmacology provides an overview of research into the involvement of glucose and insulin in the modulation of cognition and synaptic plasticity in health and in disease. In three sections, the effects of glucose and insulin on the brain will be discussed from a physiological perspective, in relation to brain ageing and Alzheimer's disease, and in relation to diabetes mellitus, respectively. These subjects will be covered from various angles, from the level of molecular neurosciences, to the level of applied neuropsychology.

Background: Diabetic conditions are associated with alterations in brain functions like memory deficits through processes like synaptic dysfunction in the hippocampus. Administering a combination of silver nanonaringenin and vitamin E appears promising since they are known to prevent diabetes and memory deficits in previous studies, and nanoformulation of naringenin may be one way to improve delivery and bioavailability of naringenin in the brain. This study investigated the effects of co-administering silver nanonaringenin and vitamin E against memory deficits and synaptic dysfunction in the hippocampus of a mice model of high-fat diet and streptozotocin (HS).

Aim: To assess the efficacy of the combined administration of myo-inositol and zinc, a mineral involved in the insulin pathway, in paediatric obesity with insulin resistance on HOMA-IR, glucose-insulin metabolism, and lipid profile.
Materials And Methods: Double-blind, randomized, placebo-controlled study conducted in North Italy. Fifty-six patients (10-18 years, Tanner stage ≥3) with obesity and insulin resistance were randomized to myo-inositol (2000 mg), zinc gluconate (5 mg), and galactooligosaccharides (GOS) from plant-based origin (1000 mg) (TRT) or placebo (PLC) containing only GOS from plant-based origin (1000 mg).

To examine the changes in obesity-related hormones and metabolic syndrome markers in male high school students with obesity following a weekend-focused moderate- or high-intensity exercise program at the recommended weekly physical activity level, or a program of regular exercise 3 times a week at moderate intensity, over a 10-week period. Forty-eight male high school students who were obese with a body fat percentage of ≥25% were randomly assigned to one of three groups: a regular moderate-intensity exercise group (n=17) that freely selected and performed moderate-intensity aerobic and resistance training exercises, every Monday, Wednesday, and Friday, for a total of 150-300 min/wk; a weekend-focused moderate-intensity exercise group (n=15) that freely selected and performed aerobic and resistance training exercises every Saturday for 150-300 min; and a week-end-focused high-intensity exercise group (n=16) that freely selected and performed aerobic and resistance training exercises every Sunday for 75-150 min. Insulin and leptin levels significantly decreased in all the groups, with the greatest reduction in the regular exercise group.
