Central hypothyroidism is characterized by either decreased TSH or, more commonly, normal TSH. This study aims to check whether this biochemical difference related to the severity of the pituitary disease, metabolic processes and general well-being. A retrospective analysis was performed on 108 inpatients with hypopituitarism, aged 18-80, hospitalized (1 January 2020, through 31 December 2022) in the Department of Endocrinology and Metabolic Diseases, Medical University of Lodz, Poland.

Article Synopsis
  • Hypopituitarism is a disorder marked by low levels of one or more anterior pituitary hormones, with notable differences in health outcomes between sexes; women experience higher rates of cardiovascular issues and higher mortality.
  • The dysfunction of the hypothalamic-pituitary-gonadal (HPG) axis leads to hormonal deficiencies (like estrogen and testosterone), and treating these deficiencies with hormone replacement therapy is crucial for health benefits.
  • Despite the positive effects of hormone replacement in reducing mortality risk, the reasons behind the sex disparities in health outcomes for hypopituitarism patients remain unclear, with possibilities including lack of aggressive treatment for women and premature loss of protective estrogen effects.

Pituitary hormone deficiency, hypopituitarism, is a dysfunction resulting from numerous etiologies, which can be complete or partial, and is therefore heterogeneous. This heterogeneity makes it difficult to interpret the results of scientific studies with these patients.Adequate treatment of etiologies and up-to-date hormone replacement have improved morbidity and mortality rates in patients with hypopituitarism.

More and more clinical evidence confirms that hypopituitary dysfunction can lead to liver cirrhosis. The causes of hypopituitary dysfunction include sellarneoplastic diseases, hereditary diseases, perinatal adverse events, etc. Clinically, growth hormone deficiency (GHD) often the first cause in hypopituitary dysfunction.

Androgen deficiency in hypopituitary women: its consequences and management.

Rev Endocr Metab Disord

June 2024

Department of Internal Medicine and Clinical Nutrition, Institute of Medicine, Sahlgrenska Academy, University of Gothenburg, Gröna Stråket 8, Gothenburg, 41345, Sweden.

Women with hypopituitarism have various degrees of androgen deficiency, which is marked among those with combined hypogonadotrophic hypogonadism and secondary adrenal insufficiency. The consequences of androgen deficiency and the effects of androgen replacement therapy have not been fully elucidated. While an impact of androgen deficiency on outcomes such as bone mineral density, quality of life, and sexual function is plausible, the available evidence is limited.
