Learnability of Ultrasound-Guided Locoregional Anesthesia for Carotid Endarterectomy.

J Clin Med

December 2024

Department of Anesthesiology, Heidelberg University Hospital, Medical Faculty, University of Heidelberg, 69120 Heidelberg, Germany.

: There is an ongoing debate about the most advantageous anesthesia technique for carotid endarterectomy (CEA). From an anesthesiologic perspective, locoregional anesthesia (LRA) appears to offer significant benefits. However, the learning curve and complication rates for anesthesiologists newly performing ultrasound-guided LRA for CEA remain unclear and are to be examined in greater detail in this study.

[Carotid endarterectomy : General versus local anesthesia, which is the best ?].

Rev Med Liege

October 2024

Service Anesthésie-Réanimation, Clinique Saint-Luc, Bouge, Belgique.

Article Synopsis
  • - The article compares two anesthetic techniques for carotid endarterectomy: cervical plexus block, which allows for better neurological monitoring, and general anesthesia, which is necessary for certain patients and provides better physiological control.
  • - It highlights the benefits of ultrasound-guided cervical plexus block as a safer alternative to general anesthesia, enhancing anesthetic options.
  • - The importance of personalized medicine in anesthesia is emphasized, advocating for tailored approaches to meet individual patient needs for optimal outcomes.

Article Synopsis
  • - Syncope is a common issue in emergency departments and can be caused by various factors, with carotid sinus hypersensitivity (CSS) being a significant one affecting quality of life.
  • - A 43-year-old man with recurrent syncope was diagnosed with CSS after thorough evaluation and treated with a stellate ganglion block over 12 days.
  • - Post-treatment, the patient showed symptom improvement and remained symptom-free during a 7-month follow-up, suggesting the need for more research on stellate ganglion block therapy as a potential treatment for CSS.

Introduction: Carotid endarterectomy is performed for patients with symptomatic carotid artery occlusions. Surgery can be performed under general and regional anesthesia. Traditionally, surgery is performed under deep cervical plexus block which is technically difficult to perform and can cause serious complications.

Objective: To investigate the effect of ultrasound-guided stellate ganglion block (SGB) on cerebral oxygen metabolism and serum S100B during carotid endarterectomy (CEA).

Methods: Patients who were prospectively enrolled to receive CEA under elective general anesthesia were randomized into an SGB group and a control group (ChiCTR2000033385). Before anesthesia, the SGB group underwent ipsilateral SGB under ultrasound guidance, while the control group did not.
