The Nijmegen inception cohort of early rheumatoid arthritis.

J Rheumatol Suppl

Department of Rheumatology, University Medical Centre Nijmegen, Nijmegen, The Netherlands.

Published: March 2004

Rheumatoid arthritis is a heterogeneous chronic disease with an unpredictable disease course. To study such a disease, longterm observational studies are needed, with regular assessment of patients using valid and reproducible measures. This article describes the purpose and design of such a study at the Department of Rheumatology, University Medical Centre Nijmegen. Within this inception cohort, several instruments to assess RA were developed and validated, such as the Disease Activity Score, the European League Against Rheumatism response criteria, and a Dutch version of the Health Assessment Questionnaire disability index. These instruments are described and results of studies are discussed concerning prognostic and predictive factors, disease course, and the relationship between the different process and outcome measures.

Background: Surgical site infections (SSIs) lead to increased mortality and morbidity, as well as increased healthcare costs. Multiple models for the prediction of this serious surgical complication have been developed, with an increasing use of machine learning (ML) tools.

Objective: The aim of this systematic review was to assess the performance as well as the methodological quality of validated ML models for the prediction of SSIs.

Article Synopsis
  • Model-informed precision dosing (MIPD) uses mathematical models to determine the best medication doses for individual patients, but challenges in its implementation prevent broader use in healthcare.
  • A systematic review analyzed 15 articles to identify 72 barriers and facilitators impacting MIPD's integration, highlighting issues such as limited data for model validation and unclear regulatory guidelines.
  • Key facilitators include the creation of user-friendly MIPD tools and collaborative efforts among stakeholders, along with educating healthcare providers to support MIPD in clinical settings.

People diagnosed with pelvic organ prolapse (POP) face preference-sensitive treatment decisions. We conducted a systematic review, meta-analysis, and narrative synthesis to determine the effect of decision-making interventions for prolapse on patient-reported outcomes. To gain a more complete understanding of all potentially accessed resources, we also conducted an environmental scan to determine the quantity and quality of online interventions for prolapse decision-making.

Article Synopsis
  • The study investigates the relationship between IL-6 signaling, particularly through the SNP rs2228145, and the risk of developing tuberculosis (TB), considering that inhibitors of IL-6 signaling might increase TB progression risk.
  • The researchers conducted a meta-analysis of various genome-wide association studies (GWAS) to analyze genetic data and extract effect estimates related to the C allele of rs2228145, which is associated with reduced IL-6 signaling.
  • Preliminary findings include data from 17 GWAS, covering a substantial sample of individuals with tuberculosis and a large control population, aiming to understand how genetic factors might influence TB susceptibility.
