Sore throat is one of the most common reasons for visits to family physicians. While most patients with sore throat have an infectious cause (pharyngitis), fewer than 20 percent have a clear indication for antibiotic therapy (i.e., group A beta-hemolytic streptococcal infection). Useful, well-validated clinical decision rules are available to help family physicians care for patients who present with pharyngitis. Because of recent improvements in rapid streptococcal antigen tests, throat culture can be reserved for patients whose symptoms do not improve over time or who do not respond to antibiotics.

This case highlights that refractory pancytopenia leading to death can occur with methimazole treatment even at a very low cumulative dose and after a very short duration of exposure. In addition, the standard treatments to correct the pancytopenia may not be effective and a bone marrow transplant may be required. Although human metapneumovirus(hMPV) infection can induce severe symptoms in older adults or immunocompromised patients, it usually causes mild symptoms in young immunocompetent adults. The prevalence of hMPV infectious disease is highest during the late winter and early summer. We report a hypoxemic case of hMPV infection in a young immunocompetent man that occurred in the first autumn after the reclassification of coronavirus disease (COVID-19) from Class 2 to Class 5.
