Gout is the most prevalent form of inflammatory arthritis in males as a result of high uric acid levels that cause deposition of monosodium urate crystals in soft tissues, bones and joints. It commonly presents as swollen, erythematous and painful joint. Necrotizing fasciitis is rapidly progressing aggressive soft tissue infection that spreads along the fascial planes sparing the skin; it is very uncommon for necrotizing fasciitis to occur as a complication of gouty arthritis.

Septic arthritis and reactive arthritis are both recognized as distinct causes of swollen joints; however, they can, at times, overlap as causes of acute polyarthritis. Septic arthritis is an orthopedic emergency, typically caused by bacterial infection, and requires urgent antibiotic treatment and joint drainage to prevent irreversible joint damage. In contrast, reactive arthritis is a sterile, immune-mediated arthritis that occurs following infections and is managed with anti-inflammatory treatments such as corticosteroids and non-steroidal anti-inflammatory drugs (NSAIDs).

Distal femoral fractures are uncommon in the newborn period, and conventional radiographs can miss the diagnosis in infants born prematurely due to delayed ossification. Hence, modalities like ultrasound may have a role in diagnosis. Herein, we report a male infant born preterm with a swollen knee due to a distal femoral fracture after a non-traumatic Caesarean section.
