Objective: This systematic review and meta-analysis evaluated the efficacy of various glottic widening procedures (GWPs) in avoiding tracheostomy in infants with bilateral vocal cord paralysis (BVCP).

Methods: A systematic literature search was conducted in PubMed, Scopus, Web of Science, and Cochrane Library from inception to July 2023, without any timeframe limitations. Studies reporting GWPs (Endoscopic anterior-posterior cricoid split, Botox injection, suture lateralization, and other procedures including laser posterior cordotomy/arytenoidectomy) in infants with BVCP were included.

To analyze the efficacy of endoscopic CO₂ laser arytenoidectomy in treating bilateral vocal cord paralysis. Ninety-five patients who underwent endoscopic CO₂ laser arytenoidectomy for bilateral vocal cord paralysis at the First Affiliated Hospital of Naval Medical University(Changhai Hospital) of Shanghai from January 2009 to December 2022 were included in this study. Among them, 59 patients underwent endoscopic CO₂ laser arytenoidectomy as their first glottic enlargement surgery(Group A), while 36 patients, who had previously undergone two unsuccessful CO₂ laser-assisted posterior cordotomies, underwent endoscopic CO₂ laser arytenoidectomy as a subsequent surgery(Group B).

Article Synopsis
  • This study investigates the effectiveness of a specific surgery (posterior cordotomy with partial arytenoidectomy) for patients with bilateral vocal fold paralysis (BVFP).
  • The analysis included 37 patients treated between January 2019 and December 2023, focusing on their breathing, voice quality, and swallowing both before and after the surgery.
  • Results showed a high success rate in improving breathing (86.5% of patients) but an initial decline in voice quality for the majority post-surgery, with improvements noted by three months, indicating the procedure's overall clinical value despite temporary challenges in voice.

Percutaneous and open anterolateral cordotomy for intractable cancer pain: a technical note.

Neurochirurgie

November 2024

Department of Neurosurgery, University Hospital of Saint-Etienne, France; NEUROPAIN Lab, INSERM U1028, University Jean Monnet, Saint-Etienne, France.

Article Synopsis
  • Anterolateral cordotomy (AL-C) is a procedure used to relieve severe cancer pain, but its usage has declined, risking the loss of expertise among neurosurgeons.
  • The study reviews patient selection, outcomes, and current techniques for both percutaneous and open AL-C, supported by case examples and visuals.
  • AL-C effectively reduces pain and decreases reliance on pain medications, although it may sometimes fail to provide long-lasting relief or can lead to mirror pain.

Abductor vocal cord paralysis (AVCP) is a disabling disorder that affects the voice and the quality of life of a patient, and hence, there is importance in providing a detailed knowledge of its aetiology and management. In this review article, we offer an implicational definition of AVCP and a discussion of its background, viewed as potentially affecting voice production and health. Finally, we explore modalities of diagnosis by medical history and physical examination, visualization techniques like laryngoscopy and stroboscopy, and radiographic imaging such as computed tomography (CT) and magnetic resonance imaging (MRI) scans.
