Neurosyphilis (NS) is an infectious disease caused by invading the central nervous system. It can manifest at any stage of syphilis, and is often misdiagnosed due to its atypical and progressive symptoms. The increasing incidence of NS underscores the necessity for early and accurate diagnosis.

This clinical guideline was designed to serve as a hospital guide in the diagnostic, therapeutic, and monitoring approach to neurosyphilis, considering whether there is positivity or not for the human immunodeficiency virus. Syphilis is a sexually transmitted notifiable disease, known as the great imitator. In around 30% of cases of untreated disease, there is a persistent central nervous system infection.

A mentally ill antenatal mother of 34 weeks gestation was diagnosed as a case of latent syphilis of unknown duration and was treated adequately with benzathine penicillin. One month after last dose of penicillin she delivered a male baby without any clinical or radiological evidence of syphilis, but reactive RPR in 1:64 dilution. Baby was treated as per CDC guidelines.

Rationale: Multiple syphilitic gummas involving both the brain and spinal cord are quite rare. Central nervous system (CNS) syphilitic gummas are commonly misdiagnosed as CNS tumors, and clinical suspicion and diagnosis of a syphilitic gumma by physicians are vital to avoiding unnecessary surgeries. Our case emphasizes the importance of routine serologic syphilis tests and standard therapy with penicillin in patients with a CNS mass.
