Purpose: Circadian rest-Activity Rhythm Disorders (CARDs) are common in patients with cancer, particularly in advanced disease. CARDs are associated with increased symptom burden, poorer quality of life, and shorter survival. Research and reporting practices lack standardization, and formal diagnostic criteria do not exist.

Background: Adequate circadian timing of cancer treatment schedules (chronotherapy) can enhance tolerance and efficacy several-fold in experimental and clinical situations. However, the optimal timing varies according to sex, genetic background and lifestyle. Here, we compute the individual phase of the Circadian Timing System to decipher the internal timing of each patient and find the optimal treatment timing.

Technical demands, economic considerations and allocation of services around the clock bring about, that night work constantly represents a portion of 12-15% in Germany. Work against the biologic clock increases the risks for accidents and may produce health risks as sleeping or gastro-intestinal disorders, depression, cardio-vascular diseases, overweight, and a disturbed sexual activity and fertility. Adaptation to an altered day/night rhythm during night shift work takes more than a week, and even then the time shift is rarely complete.

Nearly all functions of the human body are organized across the 24 hours of the day. This is also true for functions involved in the regulation of pharmacokinetics such as gastric absorption and emptying, gastro-intestinal perfusion, and liver and kidney functions. Several clinical studies, performed in a cross-over design, have provided evidence that the pharmacokinetics of mainly lipophilic drugs can be circadian phase-dependent.

The aim of this randomised, cross-over trial was to compare the gastroduodenal tolerability and anti-aggregating effect of effervescent calcium carbasalate (ECC equivalent to 160 mg aspirin) given once daily either in the morning or in the evening. Twelve healthy volunteers received calcium carbasalate for 2 periods of 5 days (21 days of wash-out between the 2 periods). The principal criterion was the gastroduodenal tolerability assessed by the total number of lesions at upper gastro-intestinal endoscopy.
